Job summary
Role Purpose:
As directed by the service, the post-holder will be
an integral member of a multidisciplinary team offering specialist Drug and
Alcohol clinical interventions to a cohort of service users in the service.
The post holder will be a Registered Nurse
RMN or RGN who
has completed the V300 Independent and Supplementary Prescriber course; and is
currently registered as an Independent Prescriber.
They
will have working knowledge and experience of both drug and alcohol
detoxification and treatment programmes with an understanding of both clinical
and psychosocial aspects of care.
The post holder will be responsible for the delivery of
community based medical support services that provide a community
detoxification service and access to medical prescribing for the treatment of
drug and alcohol dependence. They will
work closely and in partnership with the other elements of GDAS.
The NMP will take a lead role in developing
a nurse led prescribing clinic and will act as a point of contact for other
workers around prescribing issues.
The
NMP will work under the guidance of the Clinical Lead and be responsible for
the management of their caseload including screening, comprehensive
assessments, care planning and reviews, titration and prescribing services,
support through detoxification and rehabilitation, alongside other clinical
interventions.
Main duties of the job
The NMP will liaise with other service staff to
ensure that detox preparation and aftercare is appropriately arranged. Service
users will be seen at multi-agency sites and GP surgeries across the locality,
There will be a responsibility to ensure the safe and efficient
administration of controlled drugs and general healthcare medications to
service users with a responsibility for the maintenance of stock levels and
adherence to legal recording requirements.
The post holder will also be required to prescribe a range
of authorised medications to service users under the governance of the clinical
lead.
The post holder will deliver all services in line with Kaleidoscopes
Operational Policies and Standard Operating Procedures (SOPs), ensuring that care is safe, undertaken to a high standard and informed by up-to-date
practice. The post holder should therefore be committed to
evidence-based practice, sound clinical governance and continual improvement.
The post holder should enjoy face-to-face contact with
people.
They will have excellent communication skills, be able to work closely with
other multi-disciplinary agencies and flexibly and efficiently within a team,
to ensure services are delivered equitably
Individual and clinical supervision will be provided six
weekly for one to two hours as required. There will be an induction period,
during which training will be provided in specific areas.
About us
The aim of the Gwent wide Adult Drug and Alcohol Service
is to engage drug and/or alcohol users in a continuum of service provision in
order to reduce substance related harm and improve functioning in all domains
of the service users lives. This includes engaging and assessing service users,
brief interventions and harm reduction, outreach, training, case management and
aftercare, health promotion, detoxification programmes, needle exchange,
maintenance and relapse prevention prescribing and wellbeing services.
We serve the population of Gwent (including those who are
homeless) within the five local authority areas. The service is provided for
adult drug and/or alcohol users aged 18 and over and are provided in a
non-judgmental, non-discriminatory, professional manner demonstrating courtesy
and respect for all individuals.

Job responsibilities
Main Duties & Responsibilities:
To engage with people who have substance misuse problems and providing
them with advice, screening and clinical interventions to address their
substance misuse issues, and primary health care.
To carry out comprehensive assessments and contribute to the development
of individual recovery plans with those who express a wish to access community
treatment/support services.
As a nurse prescriber, to ensure that service users are fully informed
and have given consent to treatment.
To titrate patients safely on to a treatment regime (where required) and
ensure that procedures are constantly reviewed to promote best evidenced based
practice in accordance with clinical guidelines and local procedures
Ensure that service users who are dependent on alcohol are assessed for
their suitability to access a community detoxification.
Deliver community-based medical detoxes in conjunction with an
appropriate package of care, where it has been assessed as safe to do so.
Deliver a range of clinical interventions, including HCV /HBV testing
and vaccination and coordinating the onward referral of those diagnosed with a
blood borne virus.
Raising awareness of blood borne viruses, their transmission, treatment
and the significance of infection
Undertake phlebotomy and dry blood spot testing, as required, to
undertake routine screening and testing, ensuring adherence to identified
pathway for blood sample labelling, collection, obtaining results and
signposting follow-up and any next steps planning.
To offer wound care assessment and basic wound dressing to clients.
To refer and liaise with other agencies involved with the clients
broader care plan as part of the Service, including Health Services, hospitals
and GPs.
Make referrals to local specialist teams for an inpatient
detoxification, (when the service users medical history or personal
circumstances are not suitable for community detoxification).
To develop and progress community detoxification, taking the lead in
ensuring that the service has a comprehensive and open response community
detoxification.
Ensure that procedures are constantly reviewed to promote best practice
and evidenced based practice in accordance with UK Guidelines on Clinical
Management, NICE guidance, and clinical protocols and procedures.
Participate in clinical audits, service reviews and the evaluation of
service user satisfaction, implementing resulting recommendations, in order to
improve and develop the service
To maintain an up to date knowledge of medicine management and both
local and national guidelines and agreements.
To attend meetings appropriate to the development and maintenance of the
role
To adhere to Patient Group Directives and ensure requirements on
updating are maintained
To receive regular clinical supervision and maintain a current knowledge
of appropriate interventions
Emotional Effort
