The Neighbourhood Team consists of
Occupational Therapists, Community Nurses, Physiotherapists, Senior Nurse
Practitioner, Healthcare Assistants and Administrators. GP and social care
colleagues work closely alongside.
The
Neighbourhood Team operates from 7:30am to 18:30pm on a seven-day rota
including weekends and Bank Holidays. You will be supported to work flexibly
within these hours.

Main duties of the job
Your
typical working day will include triage and allocation of planned and unplanned
visit requests for occupational therapy interventions. This may involve urgent
response assessment as part of the teams admission avoidance remit, supporting
complex hospital discharges and planning domiciliary interventions to
facilitate independent living. You will work alongside occupational therapy, nursing
and physiotherapy colleagues in the same team. The role includes mentoring occupational
therapy students. We have close links with Worcester and Coventry universities
in support of these student placements.

The post holder will work as an autonomous
practitioner within an interdisciplinary Neighbourhood team providing rapid and
expert professional assessment and treatment interventions to patients either
in their own homes or other community settings to avoid inappropriate hospital
admission or to facilitate early discharge. This will be achieved through a
combination of hands on expert clinical skills as an Occupational Therapist with
knowledge to treat, educate and support patients; and by working with patients
and their families to assess, plan and implement appropriate packages of care
and treatment
Provide
case management for a designated caseload of patients who have immediate and /
or complex care needs.
Triage patients to ensure prioritization of
clinical need.
Work holistically with all team members,
carrying out joint visits and actively participating in the integration of
services increasing the knowledge of all team members through case discussion
and joint working.

Essential
- Diploma/ degree in Occupational Therapy
- Health Professionals Council registration
