Job summary

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced general practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients.

Main duties of the job

  1. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
  2. Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  3. Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate.
  4. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice
  5. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  6. Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children
  7. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person

MDC PCN comprises of 2 Practices in Mottingham, Downham and Chislehurst in London Borough of Bromley. The practices have a combined geographic area made up of around 35,000 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. The team is well supported with ongoing professional development (training support, CPD, and peer support). The role contributes to the improving quality of care of our patients across the network of practices.

Person Specification

Qualifications

Essential

  • 1.Registered with the General Medical Council (GMC)
  • 2.Having obtained the Certificate of Experience from the Joint Committee on Postgraduate training for General Practice
  • 3.Membership of appropriate Medical Organisation (MDU, MPS, etc)
  • 4.MRCGP
  • 5.Annual appraisal updates

Experience

Essential

  • -Experience in working in a primary care environment
  • -Experience of continued professional development

Desirable

  • -Experience of medicine management

Knowledge

Essential

  • -Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP
