Main duties of the job

Key Responsibilities:

  1. Conducting comprehensive consultations with patients to assess their medical needs and formulate treatment plans.
  2. Performing clinical examinations and diagnostic tests.
  3. Prescribing medications and monitoring response to treatment.
  4. Maintaining accurate and up-to-date medical records.
  5. Collaborating with multidisciplinary teams to ensure coordinated and integrated care.
  6. Participating in clinical audits and quality improvement initiatives to enhance service delivery.

Ash Grove Medical Centre is CQC rated 'Good' across all five areas.

We are a large GP practice with 13,000 patients based in Knottingley with a small branch surgery at Eggborough.

Ash Grove Medical Centre is led by four GP Partners working along side the Practice Manager, supported by a large clinical and non clinical team.

Job responsibilities

Clinical Responsibilities:

  1. As agreed in cooperative practice with colleagues, the post holder will make themselves available to undertake a variety of duties including surgery consultations, telephone/video consultation, visiting patients at home, on call, checking and signing repeat prescriptions, dealing with queries, paperwork and correspondence in a timely fashion. To include the designation share of workflow and pathology reports.
  2. Making professional, autonomous decisions in relation to presenting problems, whether self referred or referred from other health care workers in the organisation.
  3. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  4. Engaging with activities such as QOF/DES/IIF and other additional work essential for providing a full service for patients.
  5. Screening patients for disease risk factors and early signs of illness.
  6. Admitting or discharging patients to and from the caseload and referring management protocols, developing care plan for health.
  7. In consultation with patients and in line with current practice disease management protocols, developing care plan for health.
  8. Recording clear and contemporaneous consultation notes agreed standards.
  9. In general the post holder will be expected to undertake all the normal duties and responsibilities associated with GP working in Primary Care.
  10. Providing counselling and health education.
  11. Compiling and issuing computer generated acute and repeat prescriptions.
  12. Prescribing in accordance with practice prescribing formulary, or generically whenever clinically appropriate.

Other responsibilities within the organisation.

  1. Awareness of compliance with all relevant practice policies/guidelines- Prescribing, CQC, Confidentiality, data protection and health and safety.
  2. Contributing to evaluation/audit and clinical standard setting within the organisation.
  3. A commitment to learning and audit to ensure evidence-based best practice.
  4. Contributing to the housekeeping and updating of computer based patient records.
  5. Contributing to read coding patient data.
  6. To comply with the practice IT policy.
  7. Attending training and events organised by the practice or other agencies, where appropriate.

Person Specification

Qualifications

Essential

  • Fully qualified General Practitioner registered with the General Medical Council (GMC).
  • Evidence of annual appraisal and revalidation.
  • On medical performers list
  • Clean UK driving licence.
  • Complete and enhanced DBS check.
  • Previous experience working as a GP in primary care settings.

Desirable

  • Evidence of continued professional development, long terms condition training such as Diabetes, Respiratory, Coronary Heart Disease, Chronic Kidney Disease.

Experience

Essential

  • Clincal goverance delivery of QOF targets, Self audit and reflection.
  • Organised and efficient in record keeping and completion of paperwork. Excellent time management, being able to prioritise work and work under pressure.
  • Computer literacy and excelling communication skills. Good people skills and attention to detail.
  • Strong communication and interpersonal skills, commitment to providing patient centred car and promoting health and wellbeing.
  • Ability to work effectively within a multidisciplinary team.
  • Willingness to participate in continuous professional and development activities.
