Job summary

We are looking for a team-oriented GP to provide quality healthcare and outcomes for our patients, by providing 6 sessions per week, with scope for occasional non-patient facing sessions for project work.

Main duties of the job

Working autonomously, you will be responsible for the provision of medical services to the practice population, ensuring the highest standards. Supporting and supported by the clinical and non-clinical team, you'll manage a caseload dealing with a wide range of health needs.

You will have sound clinical skills, but equally important is an appreciation of the benefit of working as part of the larger team, and the desire to act accordingly. Team working creates a virtuous circle where both our patients and our work life balance benefit.

Henmore Health is a positive and dedicated provider of primary care in the Ashbourne area, the gateway to the Peak District. We comprise 2 practices located 7 miles apart Henmore Health The Surgery Ashbourne and Henmore Health Brailsford. We are high performing SystmOne dispensing practices, serving a combined growing rural community of over 13,500 patients. Operating from purpose-built premises, we hope to expand our team comprised of GPs, ANPs, Pharmacists and Pharm Techs, Nurses and a committed admin and dispensing team.

We hold full Training Practice status and can therefore offer excellent opportunities for GPs with special interests or those who wish to pursue a particular direction. We take an extremely active role within our PCN as it develops. We champion the use of ARRS team members (paramedics, physios, mental health etc) and seek to integrate them fully to maximise the benefit to both patients and our ability to serve them.

JOB SUMMARY

To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, ensuring the highest standards of care whilst complying with all practice contractual obligations. To support the clinical and non-clinical team in delivering care, managing a caseload dealing with a wide range of health needs.

The post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

The requirements of the role are constantly changing and should be kept under regular review. The responsibilities listed are not designed to be exclusive but serve to illustrate the scope of the role.

PRIMARY RESPONSIBILITIES

The following are the core responsibilities of the Salaried GP. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Provide a full range of medical services as defined in the core GMS contract and additional commissioned services, where agreed and appropriate.
  • Respond to and assess the health care needs of patients with undifferentiated and undiagnosed problems presented including history taking, diagnosis, investigation, treatment and referral as appropriate.
  • Provide effective management of long term conditions, supported by the nursing team in line with national guidelines and pathways.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health or social care workers. Speak with a GP Clinical Lead when seeking another clinical or non-clinical opinion.
  • Screen patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current disease management protocols, developing care plans for health.
  • Provide counselling and health education, and signpost to additional information and services, as appropriate and advice to patients to enable self-management, as clinically appropriate and choice.
  • Admit or discharge patients to and from the caseload and referring to other care providers, as appropriate.
  • In line with good practice, regularly review medications and monitor repeat prescribing.
  • Prescribe in accordance with the prescribing formulary (or generically) whenever this is clinically appropriate. Seek support where appropriate from medicines management team.
  • Compile and issue computer-generated acute and repeat prescriptions (avoiding hand- written prescriptions whenever possible). Check for errors and rectify immediately.
  • Provide appropriate health promotion and preventative health care advice to all patients, as appropriate.
  • In accordance to the practice timetable, as agreed, undertake a variety of duties including surgery consultations and emergencies, inclusion on any extended hours and Duty GP rota as applicable, telephone consultations and queries, triaging, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient medical reports and examinations (e.g. insurance companies) on behalf of the GP practice, referral letters (NHS and private), document management and correspondence in a timely manner.
  • Awareness of and adherence with all practice policies, protocols and guidelines.
