An exciting opportunity has arisen across two extremely well respected practices in north manchester - The Neville Family Medical Centre and Cheetham Hill Medical Centre - for a nurse associate to join our well established nursing team.
Main duties of the job
Patient Assessment: Conduct initial patient assessments, including taking medical history, recording vital signs, and performing basic physical examinations.
Patient Care: Provide direct patient care under the supervision of a registered nurse or physician, including wound care, administering medications, and assisting with minor surgical procedures.
Health Promotion and Education: Offer health promotion and education to patients on topics such as preventive care, chronic disease management, and lifestyle modifications.
Chronic Disease Management: Assist in the management of chronic diseases such as diabetes, hypertension, and asthma, including monitoring patients' progress and providing support and education.
Routine Procedures: Perform routine medical procedures such as phlebotomy, ECGs, and administering vaccinations.
Documentation and Record-Keeping: Maintain accurate and up-to-date patient records, including documenting assessments and care provided
Collaboration and Communication: Collaborate with other staff members - both clinical and non-clinical, and communicate effectively with patients and their families.
Quality Improvement: Participate in quality improvement initiatives within the practice, such as audits, reviewing protocols, and implementing changes to enhance patient care.

About us
Although two separate practices, the Neville Family Medial Centre and Cheetham Hill Medical Centre work very closely due to Dr Asad Bokhari (lead GP across both practices) and Mike Neville (Senior Manager across both practices) both lead on the strategy and operations over the two.
Cheetham Hill MC currently has over 19,700 patients; and the Neville FMC currently has just over 5,500 registered patients.
The nursing team is very well supported across the two sites with three other practice nurses, three healthcare assistants, one further trainee healthcare assistant, and one trainee nursing associate.
With a work hard ethic coupled with friendly and supportive atmosphere, our team thrives both for each other as well as for our patients.
The support our nursing team has extends also from 13 GP's, 1 ANP, 3 pharmacists, 2 PA's and over 30 people in the admin and management team across the two sites - meaning that there is always someone there to help.

Job responsibilities
Nursing Associates work in
practice, independently under the leadership of registered nurses, working
within the sphere of nursing and care, and within all aspects of the nursing
process.
Responsibilities
To assist and perform routine nursing tasks related to patient
care, such as routine BP checks, ECG, BMI and lifestyle advice, as directed and
agreed with line manager, registered general nurses and GPs.
To support patients to improve and maintain their mental,
physical, behavioural health and wellbeing by using behaviour change
interventions, such as undertaking health checks (physical and mental), smoking
cessation services.
Support the Practice Nurses with the management of patients with
long-term conditions/chronic diseases, using a holistic patient centred
approach.
Assist in and perform routine tasks related to patient care as directed
by the Practice Nurse GP or other member of the Primary HealthCare Team.
Be able to undertake baseline observations and identify signs of
improvement, report any deteriorations or concerns, e.g. pulse, blood pressure,
respirations, oxygen saturations, peak flow, spirometry, blood glucose levels,
ECG, assessing nutrition and hydration, participate in screening programmes and
Annual Reviews for example in patients with Learning Disabilities
Perform cervical smear screening on completion of additional
training as guidance RCN Position Statement on Registered Nursing Associates
(RNAs) training in cervical sample taking.
Administer medications as prescribed and in accordance with NMC
standards e.g. via nebuliser or injections using sub-cutaneous or intramuscular
route under Patient Specific Directions.
Following agreed protocols with referral to Practice Nurse or
GPs as appropriate assist in the assessment, planning, development
implementation and evaluation of programmes to promote health and well-being and
prevent adverse effects on health and well-being.
Deliver opportunistic health promotion using opportunities such
as new patient medicals and NHS health checks.
Document consultations accurately and appropriately, according
to NMC guideline
Ensure awareness of statuary and local clinical adult and
children safeguarding protection procedures, including systems of referral.
Ability to recognise signs and symptoms of abuse/neglect.
Ensure infection prevention and control guidelines are
maintained.
To be responsible for providing the appropriate nursing
evidenced based care of an allocated group of patients.
Be an accountable professional who is responsible for their
actions, working in line with NMC standards of proficiency and recognise and
work within the limits of their competency.
To work as part of multidisciplinary team using resources
efficiently.
To deliver quality care to patients and to work effectively
within the nursing team.
To be competent in carrying out a variety of clinical
procedures.
