Job summary

As part of our Service Redesign, new posts have been developed to support our high risk patients who require a fast response in order to prevent hospital admissions where possible. This is an exciting opportunity for an innovative and experienced therapist to join the countywide Worcestershire Adult Speech and Language Therapy (SLT) Service, which covers Community Hospitals, Out-patient Clinics and Specialist Teams, with the aim of providing the optimum evidence-based service for the people of Worcestershire.

Full time Countywide post (part time considered; minimum 0.6 WTE) will focus on Fast Response under the guidance of a Highly Specialist Clinical Lead, as well as working alongside experienced community colleagues. Clinically, you will provide specialist assessment, diagnosis and case management for patients with a range of communication disorders and dysphagia related to a variety of diagnoses and may maintain a small caseload.

You will be integral to the continued development of the Service into a high performing, well respected provider of timely, safe and effective Therapy for our patients, therefore we require an ambitious and creative clinician who is self-driven, and who also works well within a team.

You will be supported with excellent management and clinical supervision in this post, which is based in the north of the county.

We are happy to negotiate flexible working hours to fit with individual needs as well as fulfilling the requirements of the Service.

Main duties of the job

A typical day will involve managing a caseload of fast response referrals in liaison with the Clinical Lead, prioritising visits across the county, raising any potential concerns regarding capacity and demand. You may also maintain a small general caseload based in the north of the county, working at a Specialist level.

You will work independently in the assessment and management of patients with communication and swallowing disorders, completing clinical documentation on our electronic records system.

You will formulate individual therapy programmes targeting realistic patient-led goals, as well as offering support and education to carers/families.

You will liaise with other members of the team and MDT colleagues; support student placements; provide in service training for SLT colleagues and other professions in areas related to SLT, and participate in audits as required by the service and the Trust.

You will attend clinical and service meetings.

You will receive training and supervision, and will engage in self-directed learning.

You will work in a clinic, hospital or domiciliary setting to provide assessment, diagnosis and full case management of adult patients with communication and swallowing difficulties

Following your patient assessment, you will be responsible for providing appropriate intervention and evaluating outcomes

You will communicate complex condition-related information to patients, carers and relevant colleagues, as well as demonstrating practical procedures where appropriate

Person Specification

Qualifications

Essential

  • Attainment of degree or equivalent, giving eligibility to practice as a Speech & Language Therapist
  • Evidence of recent CPD relevant to this post
  • Attainment of formal dysphagia competencies allowing independent assessment, treatment and full case management with this client group
  • Attainment of RCSLT NQP Competencies Framework allowing full membership of RCSLT
  • Current HCPC Registration
  • Current RCSLT Membership
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer

Experience

Essential

  • Experience of assessment, diagnosis and full case management of adults with communication disorders and dysphagia, in a range of settings
  • Recent clinical experience in the areas of specialist knowledge relevant for this post
  • Experience of delivering related training to other professionals

Knowledge

Essential

  • Current knowledge of assessment, treatment and case management for adult clients with complex communication disorders and dysphagia
  • Evidence of a clear awareness and understanding of current national, local, professional and clinical initiatives and developments as relevant to this post
  • Demonstrable knowledge of the work of relevant organisations within the voluntary sector in the support of adults with acquired communication disorders and/or dysphagia
