Job summary
The Community Neurological Rehab Service have an exciting opportunity for a dynamic and skilled clinician to join our well established team as a Specialist Neurological Nurse.
As a registered nurse you will work as an autonomous practitioner, working collaboratively within an integrated multi-disciplinary team, supported by the Band 7 Clinical Nurse Specialist and Clinical Team Leader. The successful candidate will be motivated, forward thinking, enthusiastic and committed to becoming involved in all aspects of the patient pathway.
You will assist in managing a complex caseload and provide expert assessment and long term condition management for patients with long term neurological conditions (predominantly Multiple Sclerosis (MS) and Parkinson's disease). Input will be provided across the disease trajectory from education at onset through to mild and moderate complex disability. Care is delivered in patients own homes/usual place of residence and clinic bases for the adult population of Bassetlaw.
You will play a key part in the development of long term condition management within community services and ensuring commissioned targets & Key Performance Indicators are met.
Main duties of the job
To provide a defined level of specialist assessment, condition management and access to interventions for patients who present with Multiple Sclerosis (MS) and Parkinson's Disease.
To provide a realistic, timely and occasionally intensive support to patients within their own community setting.
To demonstrate active clinical and managerial leadership skills within the team, offering clinical support and advice and providing education and supervision to junior staff members and other professionals, if required.

We are looking to recruit an enthusiastic, passionate and dynamic individual to work within our neurological nursing team to support and manage patients living with long term neurological conditions. You will need to possess community nursing knowledge and have some experience of neurological care.
The team are based at Retford Primary Care Centre and cover the geographical location of Bassetlaw. The service operates Monday-Friday 8.00 -5.00pm (excluding bank holidays).
The Community Neuro Team consists of Specialist Nurses, Highly Specialist AHPs, Specialist Physiotherapists and Occupational Therapists, Speech and Language Therapists, Clinical Psychologists, Assistant Practitioners and Community Rehab Assistants.

As a specialist neurological nurse you will:
Deliver high quality, safe, effective and efficient service to meet the needs of the defined patient group through support, kindness and respect.
You must be an NMC Registered Nurse with Neurological experience and have evidence of continuing professional development, knowledge of working with people with complex physical health problems, to inform and support the clinical care pathways; preferably within a community setting although clinical skills and experience are transferable.
You will need to be highly motivated, have positive effective interpersonal and management skills and a commitment to promote recovery and rehabilitation orientated practice with the aim of preventing unnecessary and reduce hospital admissions in a variety of ways.
You will work closely with other members of the wider MDT, including our acute colleagues, GPs and other health professionals in order to maximise patients quality of life and encouraging self management.
Due to the nature of the post a full UK driving licence and vehicle for business use is required in order to travel independently across the geographical area; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
