Job summary

The States of Guernsey are looking to appoint a Band 6 Midwife, within Hospital Services. They will be directly responsible for the provision of midwifery care to mothers and their families, both in the hospital and community setting within a consultant led, team midwifery system of care, with a significant emphasis on multi-disciplinary working.The post holder will ensure accurate and contemporaneous records are kept of all care, information and advice given, of visits made and of the progress of each woman, reporting any deviations from the norm to the ward manager, obstetrician or registered medical practitioner.

Main duties of the job

The post holder will be responsible for the provision of autonomous, evidence-based, women centred midwifery care, whilst working collaboratively with the multidisciplinary team, otheragencies, representatives and service users. The integrated midwife will be required to undertake this role within both the ward and community services and will be allocated to work both day andnight shifts pattern within a rota period.

The Integrated Midwife will:

Work as a member of the maternity team, delivering midwifery care throughout thepregnancy continuum in the maternity unit or community setting to women and babies.

Be responsible for the coordination and delivery of maintenance of a high standard ofevidence based practice for all families in the community caseload.

Participate in the on-call rotas for the maternity unit.

Job responsibilities

Clinical Responsibilities:

Act as the womans advocate in promoting, and delivering the highest standard of midwifery care, ensuring all fundamentals of care are addressed, and current, relevant policies, guidelines and procedures are adhered to.

Develop flexible, high quality individual models of care.

Using evidence-based practice, assess the womans physical, psychological and spiritual needs, plan, implement and evaluate midwifery care involving the woman and her family, as appropriate.

Write reports, record observations and maintain plans of care, including electronic data storage; and other appropriate records, accurately, legibly, concisely and contemporaneously, in accordance with NMC standards.

Monitor the womans condition, interpret observations and identify deviations from normal.

Report and refer to relevant health professionals, carrying out midwifery interventions as needed.

Keep the woman and her family informed, when appropriate maintaining client confidentiality.

Ensure midwifery care is flexible to meet the diverse needs of women and their families.

Comply with HSC Guernseys policy for management and administration of medicines.

Undertake clinical skills, as per HSC Guernsey guidelines, relevant to role and the womans care.

Collaborate with the multi-disciplinary team and other service providers; ensure care is woman centred, effective and efficient.

Examine and observe the neonate, interpreting observations and identifying deviations from normal. Report and refer to relevant health professionals, carrying out midwifery interventions as needed.

Deliver and evaluate formal programmes for the preparation of parenthood in response to the womans need and incorporate health education into the womans care whenever possible.

Identification and management of at risk families within the child protection arena, devising plans of care in conjunction other health care professionals and engaging in multidisciplinary safeguarding practices.

Attend relevant multiagency meetings, develop and maintain close working relationships and practices with other health care professionals in relation to safeguarding.

Monitoring families compliance with child protection with child protection plans and writing relevant reports.

Engage with women and their families in avoiding and resolving complaints.

Organisation, Management and Leadership:

Act as a role model in the delivery of all aspects of service user care.

Take an active part in team working, innovation and practice development within both the community and hospital setting.

Be involved in the systematic review and development of maternity policies and guidelines.

Work closely with the midwifery management team to develop services for low risk midwifery provision across a range of setting, in line with current national standards, local initiatives and evidence.

Be involved in the implementation and evaluation of ongoing professional development and training for the community midwifery team.

Training and Education:

Implement and actively participate in audit programmes.

Utilise current research to inform development of the maternity services.

Undertake statutory training on an annual basis.

Promote professional development of self and others, through research based practice.

Maintain own continuing professional development needs. The post holder should be willing to work towards achieving degree status.

Act as a mentor/preceptor to preceptee midwives and other learners in the clinical area.

Develop and maintain competence in the additional skills related to the needs of the clients and patients.

Develop and maintain skills to deal with emergencies competently and facilitate skill drills within both the community and hospital setting.

Communication:

Communicate effectively with patients/clients and their significant others, taking into account their cultural background and cognitive functioning.

Communicate care options to women, providing unbiased, evidence based information regarding their risk and advantages of care plans, treatments and procedures in order to facilitate womens informed choice.

Promote and maintain effective lines of communication between management, midwifery and all members of the multidisciplinary team.

To act as an advocate for patients/clients, significant others and other staff members.

To ensure written/verbal communication complies with HSC and NMC standards.

Prepare verbal and written reports, including statistics on activity, service planning and audit.

Attend and participate in meetings.

Infection control:

Comply with policies for the correct disposal of clinical and other waste, sharps, and soiled linen.

Clean and disinfect equipment as needed.

Help to prevent the spread of infection by adopting infection control measures appropriate to the situation, and participate in educating staff, women and their families.

Evidence Based Care, Quality and Standards:

Keep up-to-date with current trends within specialty and ensure practice is evidence based. Be involved with the development, implementation and evaluation of innovations in care and service delivery, where evidence identifies the need.

Participate in setting and evaluating standards in care delivery.

Ensure clinical risk management is integral to part of the community team function. Undertake reporting of untoward incidents, participate in investigations, produce reports, disseminate findings and lessons learnt and participate in clinical audit review.

Maintain a professional approach to staff, patients, visitors and colleagues. Treat all equally; and with dignity and respect.

Use Information Technology, to improve and inform practice.

Legal and Professional Responsibilities

Adhere to HSC and departmental policies and procedures.

Provide and maintain a safe environment for yourself and others, taking reasonable care to avoid injury and co-operate with HSC to meet statutory requirements.

Provide statements regarding untoward incidents, as requested by line manager.

Maintain registration and midwifery competencies, and comply with NMC Code and related documents.

Sustain and improve own professional development by personal study, work based learning activities, and by using opportunities provided by HSC Guernsey.

Keep up-to-date with legal matters that may arise during course of duty.

Health, Safety and Security:

All employees have a duty to report any accidents, complaints, defects in equipment, near misses and untoward incidents, following HSC procedure.

Ensure Health and Safety legislation is complied with at all times, including COSHH, Workplace Risk Assessment and Control of Infection.

All information relating to women, their families and staff gained through your employment with HSC is confidential.

Working within HSC employees may gain knowledge of confidential matters which may include personal and medical information about patients and staff. Such information must be considered strictly confidential and must not be discussed or disclosed. Failure to observe this confidentiality could lead to disciplinary action being taken.
