As a Mental Health Practitioner, you'll work across Primary Care, Wellbeing Teams, and Mental Health Services, providing advice, assessment, treatment, education, and problem-solving approaches to patients. You'll manage a caseload, focusing on continuous assessment and treatment of individuals with mental health difficulties. The role involves recovery coordination, care management, and liaison with various organizations.

Main duties of the job

Core Responsibilities:

  • Provide safe and effective day-to-day operation of the service, including clinical enquiries and telephone triage screening assessments.
  • Undertake assessments, establish risk, and determine appropriate courses of action in partnership with service users.
  • Support adherence to prescribed treatments, monitor mental health medication, and provide short-term interventions.
  • Collaborate with multi-disciplinary teams, provide education on mental health problems, and facilitate access to local resources.
  • Maintain accurate clinical data, evaluate outcomes, and engage in research studies.
  • Foster partnerships between Primary Care Teams and Mental Health Services, attending PCN meetings and joint governance forums.

  • Experience of working with adults in a Mental Health setting.
  • Evidence of providing brief, effective treatment to adults.
  • Evidence of effective triage/care plan and management.
  • The desire to work in a primary care setting.

Desirable

  • Work with voluntary agencies.

Specialised Knowledge

Essential

  • Knowledge and practice of the Mental Health Act 1983 and other relevant legislation.
  • Knowledge of the NHS FYFV, NSF, and other relevant documentation.
  • Knowledge of community care.
  • Awareness of stepped-care approaches.
  • Understanding common mental health problems and their management in primary care.
  • Understanding of management of risk in primary care.
  • Understanding of the application of confidentiality in primary care.
  • Care programme approach.

Desirable

  • Training in SFT.
  • Training in Systematic Approaches.
  • IT skills.

Qualifications

Essential

  • Registered Mental Health Nurse
  • Evidence of commitment to continuing professional development

Desirable

  • ACP with prescribing for depression, anxiety etc, ability to do interim reviews, CBT sessions, and QoF.
  • Teaching and assessing qualification.
  • Qualification in therapeutic interventions.
  • Independent Prescriber.
