Job summary
As a Mental Health Practitioner, you'll work across Primary
Care, Wellbeing Teams, and Mental Health Services, providing advice,
assessment, treatment, education, and problem-solving approaches to patients.
You'll manage a caseload, focusing on continuous assessment and treatment of
individuals with mental health difficulties. The role involves recovery
coordination, care management, and liaison with various organizations.
Main duties of the job
Core Responsibilities:
- Provide safe and effective day-to-day operation of the service, including clinical enquiries and telephone triage screening assessments.
- Undertake assessments, establish risk, and determine appropriate courses of action in partnership with service users.
- Support adherence to prescribed treatments, monitor mental health medication, and provide short-term interventions.
- Collaborate with multi-disciplinary teams, provide education on mental health problems, and facilitate access to local resources.
- Maintain accurate clinical data, evaluate outcomes, and engage in research studies.
- Foster partnerships between Primary Care Teams and Mental Health Services, attending PCN meetings and joint governance forums.
