An exciting opportunity has become available for a Band 5 Community Nurse to work within the North Tyneside Recovery Partnership (NTRP) within a newly developed Older Person's pathway.

NTRP is a dedicated service for anyone in North Tyneside, who is experiencing problems with drugs and/or alcohol. The service does not have any age restrictions therefore is able to provide support to those of all ages. We believe recovery from addiction and substance misuse is possible for everyone, and we're here to help you achieve it. Our aim is to make recovery a reality in North Tyneside - whether you're dealing with problematic drug and/or alcohol use or helping support others that are

The team have strong links with a number of services both within CNTW and externally and strive to work collaboratively to deliver high quality care.

Main duties of the job

To support embedding the new Older Person's pathway

There will be an expectation of an outreach support where appropriate to support the older person whether this be in their home setting, a community base, care home etc.

The role entails working with those affected by substance and alcohol misuse providing safe and effective interventions, harm reduction advice, and use of psycho social interventions to support service users to consider and achieve recovery.

To deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and Supervisor. This will include those tailored to Addiction needs, older persons and co-morbidity.

Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.

To provide compassionate care that is based on empathy, kindness, respect and dignity.

To incorporate a client centred philosophy into nursing practice and client centred collaborative care.

In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

The post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence.

Person Specification

Education and Qualification

Essential

  • First Level Registration RMN/RNMH/RNLD Qualification

Knowledge and Experience

Essential

  • 12 months post registration experience, and completed preceptorship
  • Provide evidence based therapeutic interventions
  • Experience of working with service users with complex needs
  • Experience of working with service users and carers in successful delivery of treatment plans

Desirable

  • Knowledge or experience of working with substance/alcohol misuse
  • Previous experience within a community setting

Skills and Competencies

Essential

  • Ability to use IT system Comprehensive continuous assessment of skills Risk Assessment skills. Risk Management and Safety Planning skills. Ability to establish a rapport and demonstrate engagement skills with service users NTW/GenericPCP/NJ/1530 Adaptability, flexibility, good communication skills and a commitment to multi-agency team work Time Management skills and ability to prioritise work load Provide evidence based therapeutic interventions.
