Job summary

Bury St Edmunds Primary Care Network is looking to welcome a qualified and experienced clinical pharmacist into our PCN, to play a full and active role within our multi-disciplinary team.

The role is offered on a full-time basis, 37.5 hours per week over 5 days per week, between Monday and Saturday, on a rotation basis with one day a week at each of the PCN's practices.

Main duties of the job

  • Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas;
  • Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage);
  • Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice;
  • Providing leadership on person-centred medicines optimisation and quality improvement;
  • Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes;
  • Processing specialist treatment letters;
  • Supporting practice dispensary;
  • Processing hospital medication discharge letters;
  • Acting as a key contact, for all medication related queries;
  • Supporting improvements in clinical care, through practice-based audits and implementing change.

Bury St Edmunds Primary Care Network is a growing PCN with already existing roles including, care coordinators, social prescribers, clinical pharmacists, paramedics, mental health practitioners, health and wellbeing coaches and pharmacist technicians that support GPs and other clinical teams to enhance patient services in the local community.

Person Specification

Experience

Essential

  • Experience working as a pharmacist
  • Will hold Personal Professional Indemnity Insurance;
  • Independent Prescriber status;
  • Excellent clinical and medication knowledge;
  • Understanding and experience of working as a pharmacist in a GP practice setting, with a confident approach;
  • Experience of undertaking medication reviews;
  • The commitment to continue to learn and develop;
  • The ability to be adaptable and flexible
  • A proactive problem solver.
  • Excellent inter-personal communication skills.

Qualifications

Essential

  • Completion of pharmacy degree as well as post-graduate experience;
  • GPhC registration
