Job summary

Join our dynamic and progressive healthcare team as a trainee Advanced Nurse/Clinical Practitioner and take your career forward as a developing practitioner. As an integral member of our clinical team, you will play a crucial role in evolving our NGHT Frailty service and contribute to our exceptional care in the Emergency Department.

As a trainee AN/CP you will have the opportunity to develop under an experienced medical team identifying Frail patients, undertaking patients' clinical history, performing clinical assessments (understanding the physiology impact of ageing) develop plans of care and co-ordinate safe effective discharges. Working alongside allied services.

Main duties of the job

The Frailty team work in partnership with the Emergency Department triaging patients' ready for assessment and referral to the Frailty Unit or preparation for discharge.

The team undertake full holistic assessments providing an all inclusive comprehensive Frailty service. The trainee will be fully supported by the Frailty Team when undertaking advanced levels of clinical practice, knowledge and skills.

Job responsibilities

The post holder will have a level of extensive experience in the care of the older person. Acting within their professional boundaries and scope of practice they will be responsible for a level of assessing presenting patients, history taking, clinical assessment, assisting diagnosis, medication review and evaluation of care. The post holder will have access to Consultant Geriatric and ANP Support.

The post holder will demonstrate safe, clinical decision-making care, critical thinking and work collaboratively with the multidisciplinary team to meet the needs of the patient.

The post holder will support the delivery of policy and procedures, providing service co-ordination and leadership where required

Person Specification

Education & Training

Essential

  • Registered Nurse or Allied Healthcare Professional (NMC/HCPC)
  • Substantial post registration experience including Acute Medicine
  • Working towards or willingness to complete a MSc in Advanced Clinical Practice or equivalent portfolio of experience
  • Leadership qualification or equivalent level of education or experience
  • Teaching and assessing qualification or equivalent level of education, training or experience

Desirable

  • Experience working in A&E and or SDEC
  • Completion of a Physical Assessment and Clinical Reasoning Module

Knowledge and Skills

Essential

  • Proficient registered healthcare professional, with experience in Care of the Older Person with Frailty needs and Acute Medicine
  • Evidence of clinical experience and/or knowledge of Comprehensive/Geriatric Assessments.
  • Evidence of experience in working within a Multidisciplinary Team.
  • Knowledge of National frameworks related to Frailty and Care of the Older Adult.
  • Evidence of experience within all four pillars of Advanced Practice
  • Working within professional limitations to maintain quality of care and safe practice, identifying any clinical deterioration, and escalating appropriately in an acute area.
  • Proven record of leading and effectively managing change

Desirable

  • Previous experience working as a Clinical Nurse Specialist or Nurse/AHP Practitioner
  • Proficient in the development of protocols/guidelines

Skills

Essential

  • Effective communicator able to deliver complex and highly sensitive information to patients and relatives.
  • Promote, deliver evidence based care for patients presenting with acute, chronic and complex needs.
  • Ability to work effectively, efficiently, and safely within clinical boundaries, in clinical areas, delivering a high standard of care.
  • To be an advocate for patients' physical healthcare needs.
  • Experience of Elderly Trauma and Falls assessment.
  • Evidence of effective people management and leadership skillls.
  • A thorough understanding of the social care system and complex discharge needs regarding Frail Elderly Patients.
  • Excellent IT skills and use of patient information systems.
