Job summary

We are currently seeking an experiencedCare Coordinator, at Band 6 to work via a Bank Contract, based in Borehamwood for our Community Adult Mental Health Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

You will provide high quality, person-centred care coordination to service users with complex mental health needs under the Care Programme Approach (CPA).

The post holder will manage a defined caseload, coordinate care across agencies, assess and manage risk, and ensure delivery of safe, effective, and recovery-focused interventions in line with HPFT values and policies.

HPFT provides services for service users with mental health, learning disability and children's and adolescent mental health problems. Applicants should have some experience of working in a healthcare setting.HPFT has a range of healthcare settings across the County and therefore access to a car would be beneficial unless you have a disability as defined by the Equality Act 2010 which prevents you from driving

All applicants need to hold a valid Professional Registration and can evidence this.

Main duties of the job

Care Coordination and Clinical Practice

Act as Care Coordinator for allocated service users in line with CPA requirements.

Undertake comprehensive assessments, including mental health, social care, and risk assessments.

Develop, implement and review person-centred care plans with service users and carers.

Deliver evidence-based interventions and support recovery-focused practice.

Maintain regular contact with service users in community settings.

Support with Duty

Any other lawful assignment as may be assigned to meet the need of the service
