Working autonomously for much of the day, you will manage a defined caseload, planning visits efficiently while maintaining accurate documentation and adhering to clinical standards and protocols. You will liaise closely with hospital teams, specialist services and colleagues within CIVAS to ensure continuity of care. The role requires adaptability, clinical judgement and strong communication skills, enabling you to respond to changing patient needs while delivering high-quality care in a community setting.
