Job summary
We are looking for a proactive and compassionate Care Coordinator with strong communication and prioritisation skills, and a flexible, person centred approach, to join our supportive and welcoming team.

Are you looking for a rewarding role where you can truly make a difference to the lives of some of the city's most vulnerable and frail people? You will work as part of a multidisciplinary team, handling calls from both patients and clinicians and liaising closely with a wide range of services within the Community Frailty Team.
Your day today duties will include contacting patients by phone, managing incoming calls, processing referrals, booking appointments, and supporting the coordination of crisis care calls. You will also be involved in allocating, adding and amending patient care visits, and accurately recording information on the computer system as part of a collaborative team effort.

Main duties of the job
To provide efficient and effective administrative support to the Clinical Services and Urgent Care directorates to enable them to meet the health needs of patients, ensuring a high standard of care is delivered.
To work collaboratively across directorates providing support as required and to contribute towards a positive patient experience.
