Job summary

The Discharge Coordinator plays a vital role in supporting safe, effective, and timely patient discharges across the Trust. The postholder will work closely with ward teams, multidisciplinary professionals, patients, families, carers, and external agencies to coordinate all aspects of the discharge process and ensure a seamless transition from hospital to community services.

The successful candidate will support the planning and implementation of discharge pathways, proactively identifying and resolving barriers to discharge to minimise delays and optimise patient flow. They will act as a key point of contact for discharge-related matters, facilitating communication between healthcare professionals, social care providers, community services, and other external agencies.

The role requires excellent organisational, communication, and problem-solving skills, alongside the ability to manage competing priorities in a fast-paced environment. The postholder will contribute to improving patient experience, reducing delayed transfers of care, and ensuring effective utilisation of hospital beds while maintaining a patient-centred approach at all times.

In addition, the Discharge Coordinator will be responsible for maintaining accurate discharge records, producing performance and activity reports, supporting Continuing Healthcare processes, and contributing to the achievement of Trust, local, and national patient flow objectives.

Main duties of the job

Essential

  • Evidence of a good standard of written and spoken English through previous qualifications, presentations, or equivalent experience.
  • Demonstrated ability to learn and develop new skills, evidenced through previous study or training.
  • Educated to GCSE/'O' Level standard or equivalent qualification.
  • Evidence of continuous professional development (CPD).
Knowledge and Experience

Essential

  • Ability to create, maintain, and develop systems from which accurate information and reports can be produced.
  • Experience of producing reports on discharge data and communicating findings effectively to clinical and non-clinical staff at all levels.
  • Significant IT experience, including data extraction, analysis, and reporting.
Skills, Abilities and Attributes

  • Excellent verbal and written communication skills, with the ability to communicate effectively with a wide range of stakeholders.
  • Proactive approach to problem-solving, using knowledge, initiative, and experience to identify and implement solutions.
  • Strong literacy and numeracy skills, including report writing, customer service, telephone negotiation, and implementation planning.
  • Ability to analyse complex data, identify trends, and provide accurate reports on discharge performance and patient flow.
  • Strong organisational skills with the ability to prioritise workload and meet deadlines
