- Deliver COVID vaccinations to care home residents and housebound patients.
- Support delivery of other vaccination programmes where required (e.g. RSV, Shingles, Pneumococcal).
- Undertake pre-vaccination clinical assessments to ensure patient suitability.
- Provide clear information and reassurance to patients and carers about vaccinations.
- Monitor patients for any immediate adverse reactions following vaccination.
- Ensure accurate and timely clinical coding and documentation using SystmOne
- Work collaboratively with the wider PCN vaccination team and care home staff.
- Follow infection control, cold chain and vaccine handling protocols.
