Job summary

We are excited to offer a newly created, full-time position of Band 5 Mental Health Homeless Nurse (RMN) within the Manchester Mental Health and Homeless Team.

We would welcome applications from newly qualified nurses or existing Band 5 nurses who would like to broaden their community nursing experience.

This is the first time there has been a Band 5 post within the Manchester Mental Health Homeless Team and therefore the role will be developed alongside and supporting the post holder's development needs. The post holder will be fully supported via the trust preceptorship package if required, alongside intensive support from the senior team and wider multi-disciplinary team.

The post holder will be supported by a team of band 6 practitioners to develop their own knowledge and skills with our service users, using the combined model of assertive outreach and inclusion health. You will be closely supervised to hold a caseload of individuals within your scope of competence.

Main duties of the job

The post holder will provide engagement and assessment to clients with complex mental health and dual diagnosis needs.

The post holder will be responsible for the assessment of mental health needs for homeless individuals who are not engaged with mainstream mental health services. The role will include active participation in the service planning and future development of the mental health support for homeless individuals within the Manchester district.

The successful candidate will work to support and strengthen existing links with temporary accommodation, support services and mental health treatment services within the city of Manchester. The post holder will be supported by a well-established, specialist multi-disciplinary, trauma informed homeless mental health team and will work with individuals who are experiencing mental health problems. The role of the team is to work with a range of partnership services and homeless individuals to assess and identify the appropriate treatment pathways to address these unmet needs.
