Job summary
The North West Integrated Neighbour Team are looking for Band 6 Community Physiotherapist to join their team at Hartismere Hospital!
You will be passionate, committed, innovative, enthusiastic, and highly motivated to delivering high quality care within a patient's home.
Do you possess excellent interpersonal and communication skills with the ability to work confidently and collaboratively across professional boundaries? Are you keen and motivated to learn? If so, then your skills are invaluable to us and we would like to hear from you.
Applicants need a valid drivers licence and access to a car for community-based work.
Main duties of the job
- To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.
- To manage a caseload of patients with a range of multiple and complex health needs.
- To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
- To formulate an individual management/treatment plan for each patient using wide range of advanced assessment, clinical reasoning and treatment skills, including health coaching and promotion of self-management skills.
- To ensure interventions are evaluated and plans modified to ensure effectiveness and timely discharge for the whole therapy team.
- To provide training and advice to patients and carers to facilitate their care plan in complex situations.
- To adhere to agreed national, professional and Trust standards.

- In-depth knowledge of working within the NHS
- Able to demonstrate good IT and standard keyboard skills
- Must hold a full valid driving licence and have access to a vehicle or viable alternative
- Documented evidence of continuing professional development.
- Post registration experience in a relevant clinical setting.
- Advanced observational & reporting skills

- Evidence of previous community experience
- Able to demonstrate the management of complex casework
