Job summary

Colne Medical Centre is looking to recruit an advanced clinical practitioner (ACP) to join our multi-disciplinary team, to help us transform & modernise pathways of care, enabling the safe & effective sharing of skills across traditional professional boundaries.The role is for 3-4 days a week.

The successful candidate may come from a range of professional backgrounds. As an experienced healthcare professional, educated to masters level, they will have developed the skills & knowledge to allow them to take on an expanded role & scope of practice caring for patients.

We would love to hear from an ANP who is looking to become part of a supportive team and contribute to the continued success of our practice.

Main duties of the job

  • demonstrate a high level of expertise within the specialist service providing advice, education & support to staff, patients, families and carers.
  • be practising autonomously as an ACP within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis & treatment within an agreed scope of practice throughout the directorate.
  • support new ways of working that emphasises a more efficient & patient-focused service, & will ensure the safe treatment, referral & discharge of patients with undifferentiated & undiagnosed presentations in their area.
  • have responsibility for leading the on-going development of clinical practice & standards of care within the service, including the development of policies, procedures, protocols & guidelines in collaboration with multidisciplinary colleagues.
