Are you a people person with excellent communication skills and a passion for delivering outstanding customer service?

Horizon Healthcare is looking for motivated and enthusiastic Patient Coordinators to join our welcoming reception team. Serving a patient population of approximately 29,500 patients, we are a large, forward-thinking GP practice committed to providing high-quality care to our local community.

As one of the first points of contact for our patients, you will play a vital role in ensuring patients receive a professional, compassionate and efficient service, whether face-to-face, over the telephone, or online.

There are 3 positions available:

POSITION ONE 30 Hours over 4 days

  • Monday and Tuesday 07:45 - 15:45
  • Wednesday and Friday 10:30 - 18:30

POSITION TWO 30 Hours over 4 days

  • Monday, Tuesday and Friday 07:45 - 15:45
  • Thursday 10:30 - 18:30

POSITION Three 30 Hours over 5 days

  • Monday and Tuesday 12:30 - 18:30
  • Wednesday, Thursday and Friday 07:45 - 12:45

Main duties of the job

What You'll Be Doing

Working as part of our large and supportive reception team, you will:

Welcome and assist patients with a friendly and professional approach.

Manage appointment bookings and cancellations.

Handle patient enquiries both in person and by telephone.

Take prescriptions and administrative requests to signpost to the relevant team.

Support clinicians by facilitating effective communication with patients.

Use clinical systems and software to maintain accurate patient records.

Help ensure the smooth day-to-day running of the practice.

Horizon Healthcare is a respected and growing city centre GP practice operating across two sites caring for a diverse population of over 29,500 patients.

At Horizon Healthcare, we pride ourselves on maintaining a personal and supportive service for our patients despite our continually growing patient list. As demand for our services continues to increase, we are expanding our reception team to help ensure patients can continue to access timely support and speak directly with our team when needed.

We are a three GP Partnership supported by dedicated teams including Salaried GPs, Clinical Pharmacists, Practice Nurses and HCAs, Urgent Care Practitioners, alongside Mental Health Practitioners and Physiotherapists. These clinical teams are supported by excellent reception, administration and management teams.

Job description
Job responsibilities

  1. To deliver a quality reception service, dealing directly with patients, relatives and carers either face to face, by telephone or digitally. Reception services include general enquiries, booking or cancelling of appointments, logging of accurate information (e.g. visit requests)
  2. Facilitate efficient lines of communications between patients and clinicians at all times
  3. Extensive use of GP clinical system (SystmOne) and linked software
  4. To ensure actions requested by clinicians based on the computerised pathology reports are completed and recorded. Where indicated by the clinicians, within the agreed practice protocol, advise patients of pathology results
  5. To use and amend (where appropriate) the computerised appointment system
  6. Retrieval of information as required from both computer and manual systems
  7. Responsible for the internal and external mail distribution and delivery (this includes ensuring specimens for pathology are ready for collection)
  8. Ensure that the information resources in the practice premises are regularly updated and kept in a tidy and orderly manner
  9. Responsibility for opening or closing and securing of the building at the start or end of the day
  10. Observe a strict code of confidentiality at all times
  11. To work within the practices agreed policies and guidelines where appropriate commenting on and contributing to changing such policies
  12. Attend meetings if required
  13. To do administration duties as allocated by the Reception Manager & Management Team
  14. Providing cover at the branch surgery in Mackworth as & when needed.
  15. To assist in the annual QOF indicators i.e. smoking, booking appointments etc.

You are expected to be flexible in the duties you carry out. The Practice Management reserves the right to vary your duties from time to time as it sees fit.
