Job summary
Were looking for a Independent Assessor and Prescriber to join our exciting ADHD service for 37.5 hours per week. This is a fantastic opportunity to make a difference from the outset and play an important role in helping us develop and deliver the service.

Main duties of the job
As an Independent Assessor & Prescriber you will work as part of the PCS ADHD service working across the interface between primary care, voluntary sector and secondary services, with the aim of improving the mental health and well-being of our communities.
You will support individuals to move form surviving to thriving by helping people to feel in control of their health and wellbeing, by linking service users with health and social groups and pathways to address the impact of living with undiagnosed ADHD symptoms.
You will be integrated as part of the primary care ADHD infrastructure, providing ADHD Assessment, Diagnosis and prescribing treatments.

Job description
Job responsibilities
Clinical Practice
- Delivering structured diagnostic assessments, including history, impairment, co-morbidity and risk, and providing clear feedback and written plans.
- Conducting structured follow-up reviews to monitor treatment response, adjust care plans and support safe, effective long-term management.
- Working with the pharmacy team to initiate and adjust medication within the local formulary, including arranging baseline assessments and ongoing physical health monitoring.
- Conducting brief titration contacts until the patient is stable and completing safe shared-care transfers to primary care using the Electronic Prescription Service and relevant templates.
- Documenting clinical activity promptly and accurately, producing high-quality letters on the day and maintaining accurate, auditable records.
- Maintaining high standards of care in line with Local and National standards, professional requirements and your scope of competence.
- Using professional judgement to manage complex or unpredictable situations, recognising when guidance, support or escalation is required.
Leadership & Service Development
- Presenting complex primary care cases at the weekly MDT and contributing to audit, incident learning and quality improvement.
- Supporting the induction and development of colleagues through case discussions, day-to-day coaching and effective teamworking.
- Supporting the wider multi-professional team through leadership tasks and contributing to the development of procedures and service improvements.
- Providing activity, outcome and KPI information as agreed with the Service Team Manager and Head of Service.
- Using IT effectively and contributing to new IT tools and reporting systems, including the emerging use of AI.
- Upholding safeguarding, consent and capacity, and information governance requirements, while demonstrating resilience and ensuring concerns are appropriately escalated.
