Brockwood Medical Practice is looking for an enthusiastic and motivated Practice Nurse to join our expanding multidisciplinary team.
This is an exciting opportunity for either a newly qualified or experienced nurse who is passionate about delivering high-quality, patient-centred care in general practice and keen to develop their clinical knowledge and skills.

Main duties of the job
The Practice Nurse will provide high-quality, safe and effective nursing care to patients registered with the practice. The Nurse should be able to demonstrate, appropriate to their level of training, experience and competency:

Long-Term Condition Management
Undertake assessment, monitoring and review of patients with long-term conditions, including
Diabetes
Asthma
COPD
Cardiovascular disease
Hypertension
Chronic kidney disease
Atrial fibrillation
Stroke/TIA
Other conditions as appropriate to the nurse's competencies and practice requirements
Duties may include
Clinical assessment and review
Blood pressure monitoring
Pulse and rhythm assessment
