Job summary

The Hambleton and Richmondshire community mental health team for older people are looking to recruit a highly motivated band 3 Community Support Worker to join our multi-disciplinary team.

We are looking to recruit Community Support workers to form an integral part of our service, working under the supervision of the multi-disciplinary team to support us in delivering a high-quality service to the local population, across both Hambleton and Richmondshire.

This is an exciting opportunity to support people with both functional and organic mental illness in their own homes and care homes.

Full time and part-time hours are available

Main duties of the job

The Band 3 Community Support Worker is an essential clinical role required to deliver care as delegated by registered clinicians, including supporting group work. The post holder will contribute to the provision of mental health care support through the monitoring of both mental and physical health and in delivering non-pharmacological interventions such as behaviour support. The post holder will also be required to take physical investigations including venepuncture and E.C.G's as part of medication monitoring.

The CSW's have an essential role to play in supporting Care Homes and to ensure positive working relationships and effective joint working between other Voluntary, Community and Social Enterprise Sector services.

While experience in mental health would be an advantage, training and development opportunities will be provided in these areas for the right applicant.

The successful candidate must hold the skills to work within a team effectively and must also be able to work independently using their own initiative.

The Hambleton and Richmondshire Community team is a multi-disciplinary team that includes a range of professionals from different disciplines, skills and backgrounds.

Job responsibilities

The CSW will link directly with Duty and Key Workers, will need to respond promptly with low-level advice and interventions to reduce carer stress, and escalate concerns for deteriorating patients to prevent care breakdown and support the team to deliver prompt care that can help to avoid unnecessary hospital admissions. They require extensive knowledge regarding community resources for patients, families and carers to support individuals to stay connected. CSWs also underpin the delivery of the teams Physical Health Interventions. They provide timely physical interventions such as phlebotomy and ECGs, ensuring safe monitoring during medication changes and early detection of health concerns. This role is fundamental to the smooth operation of the MDT and the delivery of core clinical pathways.

It would be beneficial for the candidate to have completed the Diploma in Health and Social Care Level 3. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

If the successful candidate does not have the required qualifications, they will be expected to enrol on the Healthcare Assistant Framework which will include the acquisition of the level 3 Diploma Health and Social Care, level 2 in Literacy and Numeracy or equivalent to achieve the competencies. You will have a maximum of 2 years to complete this.

Your qualification must be accredited (officially recognised). This gives assurance that your qualification has met standards set by external regulators. The awarding body (examinations board) must be recognised by either Ofqual (for England); CCEA (for Northern Ireland); Welsh Government (for Wales) or SQA (for Scotland). Some examples of the awarding bodies we use/accept for Health and Social Care are: City and Guilds; NCFE; Cache; NOCN; Pearson.
