Job responsibilities

Coordinate and manage the Virtual Services team and ensure good working relationships and systems with other agencies.

To use high level communication skills to provide and receive complex information.

To maintain a professional attitude at all times and develop the role of the staff to the highest professionalstandards.

Provide advice and information to staff ensuring consistent high standard of communication.

The post holder will be responsible for creating and maintaining a learning environment that ensures that the educational professional and practise development needs of the staff are met. The role entails working across organisations and links with social services and Primary Care Teams and is key in working with family and carers to support the safe community-based care.

A full Driving license is essential for this post

• Significant post registration experience in an acute trust or community setting
• Experience of working in an MDT and across organisational and professional boundaries
• Experience of leading and managing change
• Experience of partaking in clinical audit and using outcome measures.
