Job summary

Join our District Nursing team at The Medical Centre, Woodfield Road, and help deliver high-quality, person-centred care to people in their own homes.

We are looking for a compassionate and motivated Health Care Assistant who values dignity, diversity and holistic care.

To succeed in this role, you will need:

  • NVQ Level 3 in Health and Social Care
  • Relevant experience in an acute or community healthcare setting
  • NHS experience
  • The ability to work effectively as part of a team and support people with a range of care needs

Community healthcare experience is desirable, and training will be provided to support your development.

You will join a supportive and collaborative team with opportunities to learn, develop your skills and progress your career.

Patients are visited locally and a driving licence is not required.

Main duties of the job

As a Band 3 Health Care Assistant in the District Nursing team, you will support the delivery of safe, high-quality care to patients in their own homes.

Your main responsibilities will include:

  • Providing person-centred care in patients' homes, working within your role and competencies
  • Supporting people with long-term conditions to maintain their independence and wellbeing
  • Working closely with registered nurses and other colleagues to meet individual patient needs
  • Contributing to a respectful, inclusive and high-quality community nursing service
  • Communicating effectively with patients, carers and colleagues
  • Developing your knowledge and skills through training and professional development

  • Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or A recognised equivalent course
  • Willingness to undertake further training as appropriate
Desirable
  • GCSE grade A-C or equivalent in English and Maths ECDL certificate
  • Registration with the national regulatory body (when it becomes available).
Experience

  • Experience of working in a health or social care setting
  • Experience of working as part of a team
  • Experience with the use of Microsoft Office and a variety of databases.

  • Experience with the NHS patient information database (Rio, CIS)
