Job summary

We are seeking a Band 5 Learning Disability Nurse to work within the Complex Health team in Portsmouth, within the Integrated Learning Disability Service.

This is a busy and well-established team supporting the health care needs of people with a learning disability.

You will play a vital role as part of a forward thinking and innovative team. You will receive regular support and supervision and opportunities for professional development.

As an enthusiastic and well-motivated professional, you will manage a clinical caseload involving person centred assessment, clinical nursing intervention and co-ordination of services.

You will need to hold a nursing qualification with relevant experience in supporting people with Learning Disabilities.

You will need to have excellent communication skills and to be able to gain good rapport with people from a range of backgrounds. Good organisational skills and clinical assessment/problem solving skills are also required. Collaborative working with a range of professionals is an important part of this post and it is essential you can work as part of multi-disciplinary teams.

You will receive an induction program, regular supervision, access to training and preceptorship, if appropriate.

Main duties of the job

*To work as part of the Complex Health Team (CHT) to provide high standards of effective health care for people accessing the service and / or in receipt of Continuing Healthcare funding.

*To act as a named worker to those allocated to you, and a member of the CHT, in leading and supporting the development of an inter-disciplinary approach to care, within both the supported living services and with outreach service users living in the community. Co-ordinating their care, facilitating the development, and delivery, of support plans, and annual reviews.

*To monitor and review packages of care as appropriate in line with annual reviews, or as required.

*To monitor and support complex healthcare packages that are responsive to changing health needs.

*To care plan develop/implement/evaluate programmes of care and carry out nursing procedures.

*Provides direct nursing care for patient/clients to achieve best health outcomes.

*Ensures use of nursing knowledge and skills to implement and evaluate patients/clients/family/ health needs as well as actively promote health and wellbeing.

*Evaluate clinical outcomes and reports variance to clinical pathway, protocols, guidelines.

*To direct complex inter-disciplinary team responses e.g. health action planning. This may include the involvement of outside agencies.

*Contributes to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs.
