Job summary

The PCN Neighbourhood Lead plays a pivotal role within the Primary Care Network (PCN) helping to ensure that patients receive well-organised, person-centred, and proactive care. Serving as the Strategic Lead for the PCN for the development and delivery of the Sherborne Integrated Neighbourhood, working together to provide best service without overlap.

Main duties of the job

v Identify patient cohorts, that align with the contractual obligations of the PCN and practices. Supporting PCN projects

v Be responsible for data quality

v Daily use of Microsoft Office applications (Word, Excel and PowerPoint) to support service delivery and compile reports

v To be able to work autonomously using excellent prioritisation skills to respond flexibly to competing demands and unexpected pressures

v You will help to maintain clinical records using SystmOne, support adherence to the PCN DES

v Acting as the lead contact for PCN volunteers, maintaining contact details and tracking and arranging any required training

v Managing and publishing content, via the PCN Website, Facebook and Instagram

v Be the PCN lead for the Dorset intelligence and insight Service (DiiS)

v Organising Community Events such as health fairs, outreach projects and drop-in clinics

v Scheduling PCN PLT training sessions and participating in outreach events and forums

v Providing vital administrative support to the PCN Operational Manager, the post holder must have good organisational skills

v Effective communication with patients, colleagues and professionals is an essential part of the role

About us

The successful candidate will work across the PCN with our GP's, Nurses, Pharmacy Team, Mental Health Practitioners and other healthcare staff to provide proactive and ongoing care to our 25,000 patients. The Sherborne Area PCN has three GP Practices working closely together to support good health and wellbeing in our conmmunity.

Job responsibilities

Acting as a principal point of contact for internal and external stakeholders, across health, social care and VCSE partners. Building and sustaining strong, effective relationships that support neighbourhood delivery and improved population health outcomes. Helping to facilitate an effective multi-disciplinary across organisations, breaking down organisational boundaries and promoting a collaborative neighbourhood delivery of services.

This role focuses on enabling the PCN to deliver high-quality care in line with NHS Englands Enhanced Services, and Integrated Neighbourhood Team objectives. The post holder will join a multi-disciplinary workforce that continues to improve patient care across our three practices.

Working with a wide range of partners to proactively build, cultivate, and sustain effective partnerships and professional networks, using proven communication skills. This will include representing the PCN at meetings, liaising with hospitals and community services. Supporting the PCN Operations Manager with administration, including HR, finance and performance data. Managing the PCN TeamNet portal to ensure policies and data are current. Drafting or updating relevant SOPs, contributing to continuous improvement initiatives.

Responsible for the PCN Website, ensuring legal compliance and that information displayed is up to date and encourages participation in national screening programmes. Managing the network social media accounts to promote all health-related content and PCN services.

Be aware of and communicate, both local and national health care campaigns. Promoting via websites and other media, ordering relevant resources, with responsibility for improving our patients access to digital resources. Promoting partner services, such as Pharmacy First, Home Monitoring Apps and increased use of the NHS App. Where appropriate, assist people to access self-management health education courses, peer support or interventions that support them in their health and wellbeing.

You must be able to prioritise your workload and have a flexible and proactive approach to work. This role covers several areas of responsibility and will suit a candidate who can adhere to deadlines and adapt to tasks and priorities as they arise.

The post holder must maintain confidentiality, comply with company policies, and support safeguarding and clinical governance standards. The role includes a commitment to completing any additional training required to support safe, effective patient care.
