Job summary

Cumberland House Surgery is currently looking to recruit an Advanced Nurse Practitioner to add to our existing clinical team. The post is intended to be 32hrs per week worked over 4 days but flexible for suitable candidates.

We are an established Practice looking after approximately 9500 patients over a wide geographical area. We are a forward thinking and innovative GMS Practice, situated in the thriving market town of Stone.

Main duties of the job

You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat or refer patients.

Carry out consultations with patients taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis.

Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate.

Provide clinical advice and mentorship to junior clinical staff.

Travel to patient homes and nursing homes and subsequent management of clinical problems, both acute and long-term.

Long term conditions management in the Practice setting, in accordance with established protocols based on current clinical guidelines.

Liaison with patient relatives where appropriate.

Be able to work within the latest NMC Code of Professional Conduct and to be conversant with the scope of professional practice and other NMC advisory papers.

Management of acute medical conditions, within the scope of the role holders experience and competence, with support from doctors when required

Liaison with the GP on-call whenever appropriate

Travel to patients homes and nursing homes and subsequent management of clinical problems, both acute and long-term

Long-term conditions management in the Practice setting, in accordance with established protocols based on current clinical guidelines

Liaison with patients relatives where appropriate

Refer patients to other services and healthcare providers when clinically required.

The candidate should

Registered General Nurse (Currently registered NMC)

Recognised NP qualification at minimum of degree level or equivalent

Independent Nurse Prescriber

Teaching / Mentoring experience and /or qualification

MSc in nurse practitioner studies or equivalent

Experience:

Minimum of 5 years post registration experience

Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the NP role

Experience in management of long term conditions e.g. asthma, COPD, diabetes, CHD.

Evidence of working autonomously and as part of a team
