Job summary

An exciting opportunity has arisen for an experienced and enthusiastic Practice Nurse to join Elmham Group of Practices on a 12-month fixed-term contract to provide maternity cover.

Working across our North Elmham and Swanton Morley practices, the successful candidate will join our friendly and supportive clinical team, providing high-quality, patient-centred nursing care to our practice population.

We are looking for a confident and motivated Registered Nurse with a broad range of Practice Nursing skills. Experience and competency in asthma reviews and asthma management is essential for this role, alongside the ability to support patients with other long-term conditions.

The successful candidate will be comfortable working both independently and as part of our wider multidisciplinary team and will share our commitment to providing an excellent standard of care to our patients.

Flexibility to work across both practice sites will be required according to clinical need.

Main duties of the job

The successful candidate will undertake a varied Practice Nurse role, including:

  • Asthma reviews and ongoing asthma management
  • Long-term condition reviews
  • Cervical screening
  • Child and adult immunisations
  • Dressings and wound care
  • Contraception reviews
  • Injections
  • Travel health
  • Phlebotomy where relevant to ongoing patient care
  • Participation in vaccination campaigns
  • Providing telephone advice and support to patients where appropriate
  • Supporting GPs with minor operations and LARCs as required
  • Supporting the Practice in achieving QOF and other quality targets
  • Participation in PCN Enhanced Access hours on a rota basis
  • Maintaining accurate and comprehensive electronic patient records
  • Maintaining up-to-date mandatory training, including Basic Life Support and Anaphylaxis

The post holder will work autonomously within their scope of practice whilst working closely with GPs, Nurse Practitioners, Practice Nurses, HCAs and the wider practice team.

The post holder will work as part of the wider Elmham Group of Practices team across North Elmham and Swanton Morley.

The post holder will:

  • Work as a self-directing practitioner within the managerial structure laid down by the Practice, delivering nursing care to the Practice population and working with GPs and other clinicians to meet patients' medical needs as appropriate.
  • Ensure that care delivered to the Practice population is relevant, of a high standard and clinically effective.
  • Work as a member of the Nursing Team alongside GPs, Nurse Practitioners, Practice Nurses, HCAs and the wider multidisciplinary team to provide an optimum level of care to patients.
  • Work within the framework of the NMC Code and professional standards.
  • Support audit activity and quality initiatives, including QOF.
  • Maintain and develop their professional knowledge and clinical competencies.
  • Be flexible in providing clinical cover across North Elmham and Swanton Morley according to Practice requirements.

Clinical Practice

The post holder will:

  • Provide clinically appropriate care and treatment to the Practice population, either independently or in conjunction with a GP or other appropriate clinician, in accordance with Practice policies and protocols.
  • Undertake asthma reviews and provide ongoing asthma management. The post holder must be competent and experienced in this area.
  • Undertake a range of other Long-Term Condition reviews within their scope of competence, supporting the Practice in achieving relevant QOF indicators.

Be proficient in core Practice Nursing skills including:

  • Cervical screening
  • Dressings and wound care
  • Child and adult immunisations
  • Travel health
  • Contraception reviews
  • Injections
  • Participate in vaccination and immunisation campaigns.
  • Undertake phlebotomy where relevant to a patient's ongoing care.

  • Ensure an appropriate entry is made in the patient's medical record following each consultation or clinical interaction.

Continuing Professional Development

The post holder will:

  • Undertake continuing education and training to ensure their clinical practice remains current, evidence based and effective.
  • Maintain all required statutory and mandatory training.

  • Contribute to the education and training of students and other members of the team where appropriate and within their areas of expertise.
  • Participate in audit, research and quality improvement activities where appropriate.
