Job summary
We are looking for an experienced, organised and dedicated CHC Senior Clinical Review Nurse to join our Continuing Healthcare Team here at Cheshire & Wirral Partnership Trust.
This is an excellent opportunity for a motivated clinician with a strong understanding of Continuing Healthcare, All-Age Continuing Care and the relevant National Frameworks to play a key role in the service delivering high-quality assessments, reviews and recommendations that support safe, person-centred and evidence-based decision-making.
The role requires strong clinical judgement, excellent communication skills and the ability to work collaboratively with colleagues and multi-disciplinary professionals within the service and other agencies to meet key performance indicators, improve outcomes and maintain quality, consistency and assurance across the service.
Main duties of the job
Main Duties & Responsibilities
- To use expert knowledge of National Continuing Healthcare and Funded Nursing Care Framework and Mental Health Act (117 aftercare) to ensure appropriate referrals to the Service;
- To provide patient focused service by ensuring prompt and accurate and effective decisions within the single point of access of NHS Continuing Healthcare Services;
- To use Who pays - Determining Responsible Commissioner guidance to determine appropriate patient referrals;
- Responsible for the verification of Continuing Healthcare decisions on behalf of the ICB, in line with operational procedure and organisational policy;
- Overseeing the management resolution of complaints, appeals and disputes, ensuring operational staff complete to a high standard.
- Lead the resolution of Responsible Commissioner disputes with out of area ICB's or other organisations; liaising with legal and Senior ICB leadership when appropriate.
