We have an exciting opportunity for a Health Navigator - 18 month contract to join our primary care team with a focus on Early Days in Custody, at HMP Winchester, a category B Prison located in central Winchester.
HMP Winchester is a Category B Local remand prison located in Central Winchester accommodating approximately 670 male prisoners.
There is onsite parking for all staff and it is a short walk from Winchester train station.
You will add significant value to the experience of the prisoner, prison staff and to reducing the risk of self-harm and suicide alongside wider impacts on families. This role will provide reassurance, guidance and signposting to health and social care services that will contribute to minimising distress and risk.Hours
37.5 hours a week working Monday-Friday, 9am-5pm. No weekends.
Salary
This position is full time, youll receive an annual salary of £33,000 per annum - depending on experience.

Main duties of the job
As a Health Navigator your responsibilities will vary. You will:
- Bridge HMPPS and healthcare providers to enable improvements in joint care.
- Improve identification of risk factors during early days in custody
- Offer a visible presence in the First Night Centre / Induction Wing
- Address vulnerabilities associated with early days in custody inclusive of reducing self-harm
- Create greater awareness of healthcare and social care options and the range of services available
- Support access to those services i.e., use of PODS, application process especially for people from diverse backgrounds or with specific needs
- Improve partnership working between services particularly social care, Safer Custody teams
- Improve engagement rates into healthcare and reduce did not attend appointments
- Assist with improving patient choice regarding healthcare interventions.
- Further support social prescribing and promote and manage Health Champions.
- Set up and run specific patient forums particularly relating to post ACCT
- Input into complex case meetings

Qualifications:
- Qualified RGN and experience in primary care
- Experience of setting up and implementing internal processes & procedures
- Ability to deal with confidential and sensitive issues when liaising with team members / other professionals
- Ability to demonstrate an understanding of change management & new ways of working
- Ability to manage complex & challenging behaviour
- Pro-active, engaging, confident in running forums, sharing information & knowledge
