Job summary

Widnes Highfield Health GP Federation and WA8 Collaborate Primary Care Network (PCN) are seeking to recruit an experienced and highly motivated Clinical Pharmacist to join our well-established and friendly team.

This is an exciting opportunity to work alongside an experienced team of practice-based Clinical Pharmacists, Pharmacy Technicians and wider multidisciplinary colleagues, supporting the delivery of high-quality care across Widnes.

The post will be directly employed by Widnes Highfield Health GP Federation, while working collaboratively with practices across WA8 Collaborate PCN. The successful candidate will support the delivery of the Primary Care Network DES, local prescribing priorities such as the Prescribing Initiative Scheme, and our innovative at-scale hub model clinics delivered from Highfield Hospital in Widnes.

We are committed to supporting the successful candidate's continued professional development and career progression. The role offers a salary of £53,000-£56,500 per annum, depending on experience pro rota for 30 hours per week, together with NHS Pension, a discretionary bonus and other company incentives.

This is a permanent position with the potential to develop into a more senior role with additional leadership responsibilities as our services continue to grow.

Main duties of the job

As a Clinical Pharmacist, you will work as part of a multidisciplinary team committed to improving health outcomes for our local population. There will be an opportunity to contribute to the development of new services, schemes and projects across the Federation and PCN.

You will provide expert professional advice on medicines optimisation and governance, including prescribing safety, drug monitoring and Patient Group Directions. You will help resolve medication-related queries, undertake structured medication reviews and work closely with the wider multidisciplinary team to improve patient outcomes and access to healthcare.

The role will also support the development and delivery of long-term condition services, including clinics for heart failure, chronic kidney disease, frailty and other areas of integrated care.

Delivering integrated care clinics, Structured Medication Reviews (SMRs), Extended Access services, frailty clinics and ADHD clinics.

Conducting and overseeing SMRs for patients with complex polypharmacy, including patients living in care homes.

Working closely with the PCN GP Prescribing Lead to support town-wide prescribing programmes and improvement initiatives.

Providing medicines-related advice and support to patients and healthcare professionals.

Supporting and shaping the development of integrated care and long-term condition services.

Job responsibilities

The following are the core responsibilities of the Clinical Pharmacist. From time to time, there may also be a requirement to undertake additional duties depending on service needs, workload and staffing levels.

  • Practise as an Independent Prescriber within your professional scope of practice.

  • Provide medication review services through telephone consultations, clinical record reviews, face-to-face appointments and domiciliary visits, including Structured Medication Reviews for patients living in care homes.

  • Reconcile medicines following discharge from hospital or outpatient appointments, including appropriate deprescribing.

  • Undertake targeted medication reviews for high-risk patients.

  • Review and implement safe prescribing systems for vulnerable and housebound patients, supporting improved medicines adherence where appropriate.

  • Provide advice and support to patients with self-limiting conditions.

  • Review medication for newly registered patients.

  • Implement and maintain robust repeat prescribing systems, including electronic repeat dispensing.

  • Re-authorise repeat medication within scope of practice.

  • Identify patients suitable for repeat prescribing and liaise with relevant clinicians where required.

  • Support clinicians in the management of patients with drug and alcohol dependencies.

  • Signpost patients appropriately to the most suitable healthcare professional or service.

  • Support the achievement of QOF targets, including delivering long-term condition review clinics in individual or group settings.

  • Administer influenza and other appropriate vaccinations where required and within competence.

  • Support delivery of the requirements of the PCN DES, including participation in Extended Access services.

  • Maintain accurate records of work undertaken across practices and support GP practice teams with the monitoring and management of prescribing budgets.

  • Provide leadership in person-centred medicines optimisation, including supporting antimicrobial stewardship and ensuring prescribing is in line with local guidance.

  • Contribute to quality improvement, the Quality and Outcomes Framework, prescribing initiatives and enhanced services.

  • Take a central role in the medicines-related aspects of shared care protocols and clinical research.

  • Maintain and develop professional competence and clinical expertise, keeping up to date with current therapeutic evidence, national and local policy, legislation and service developments.

  • Agree professional objectives and a personal development plan and participate fully in the appraisal process.

  • Support organisational governance processes, with particular responsibility for ensuring relevant safety alerts and MHRA guidance are reviewed and acted upon in a timely and safe manner.

  • Excellent written and verbal communication skills.
  • Ability to plan, manage, monitor and review medicines optimisation issues, particularly in relation to long-term conditions.
  • Good IT skills.
  • Ability to obtain, interpret and analyse complex clinical and technical information.
  • Ability to recognise clinical priorities, identify deviations from expected patterns and escalate appropriately to senior clinicians or GPs.
  • Ability to work effectively under pressure and meet deadlines.
  • Ability to influence, motivate and gain agreement for recommendations, including where there may be significant barriers to change.
