Job summary
We have an exciting opportunity within the Durham Resilience Team for a Mental Health and Wellbeing Practitioner (MHWBP).
The County Durham Resilience Team is a multi-agency partnership providing accessible wellbeing and mental health support to individuals and families across County Durham. Working as part of the County Durham Community Mental Health Transformation Programme, we are committed to delivering person-centred, recovery-focused support within local communities.
We are seeking an enthusiastic and motivated Mental Health Wellbeing Practitioner to join the team and deliver psychologically informed wellbeing interventions to people experiencing mental health difficulties. Working alongside service users, carers, partner organisations and multidisciplinary colleagues, you will support individuals to improve their wellbeing, achieve personal goals and maintain recovery.
The successful candidate will have either a graduate certificate (Level 6) or postgraduate certificate (Level 7) delivering the national curriculum for Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health).
Main duties of the job
As a Mental Health and Wellbeing Practitioner, you will work under supervision of an Applied Psychologist as part of a dynamic integrated mental health team alongside our Pioneering Care Partnership colleagues. You will be delivering psychologically informed interventions and supporting adults to receive the right care through care planning, including group work as directed by the wider team.
We work as part of a wider collaborative with the Pioneering Care Partnership delivering high quality care and support as early as possible to those who need it, making a definite shift form treatment to prevention.

The role of the Mental Health Wellbeing Practitioner is to develop and practice psychologically-informed interventions under supervision, working directly with adults in the community . You will support collaborative care planning, alongside other members of the multi-disciplinary team.
The postholder will carry out relevant risk assessments and risk management with the multi-disciplinary team and any patients they work with.
You will engage calmly and with sensitivity and empathy to support service users in highly distressing or emotional circumstances.
You will engage with and gather information from service users, relatives and patient records to develop a formulation.
You will work in collaboration with the individual service user and their family and carers as appropriate to develop care plans that are focused on strengths and are outcome based.
