Shared Care Haemodialysis Sister/Charge Nurse

We are offering an exciting opportunity to promote "Shared Care" to patients and staff, as a Band 6 Sister/ Charge Nurse. As the lead you will be passionate about the Shared Care concept and provide specialist clinical knowledge, advice and support to other healthcare workers, patients and their families. You will be responsible for the training and educating of both renal patients and staff across all sites within the renal directorate in the "Shared Care" concept.

You must have:

A healthcare degree

Teaching and Assessing course

Relevant Post graduate qualification

Extensive haemodialysis experience

Experience of teaching shared care/ self- care to patients

Main duties of the job

To be professionally accountable and maintain the standards of professional practice as set by the appropriate regulatory body applicable to your profession.

Treat and care for patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.

To provide specialist clinical knowledge/advice/support to other healthcare workers. Be responsible for the training and educating of both renal patients and haemodialysis staff in the Shared Care concept.

Assess, plan, implement and evaluate the specialist Shared Care programme for individuals with life limiting illness, and their carers. Liaise with other health care worker's providing specialist care to this group of patients.

Be accountable for:

  • ensuring safe and effective practice
  • enhancing the patient's experience
  • ensuring effective contribution to the delivery of the organization's objectives

Utilise clinical proficiency in delivering a high standard of nursing care. Provide managerial and leadership skills to support the Senior Sister / Charge Nurse and nursing team to deliver continually improved standards and efficient and effective use of resources.

Expert Practice

  • To practice within the NMC Code of Professional Conduct.
  • Provide expert nursing advice in the field of the specialism.
  • Provide guidance in the management of patients through protocol/practice guidelines to support nurses decision making in clinical practice.
  • Make changes to management plans of care using evidence based research and within protocols.
  • Develop efficient care pathways for both staff and patients to enable participants to achieve seamless care.
