Job summary

The Care Home Advanced Clinical Practitioner (ACP) will operate as an autonomous, independent and highly skilled clinician within the Primary Care Network. Who will manage the day-to-day operations of the Care Home team which covers both reactive care and proactive care for residents living within care home settings. The post holder will predominantly provide rapid assessment and treatment of unwell residents and provide clinical and management support as required to the proactive side of the team.

Main duties of the job

Using advanced clinical assessment, diagnostic reasoning and independent prescribing skills, the ACP will assess, diagnose, treat and manage residents with acute, chronic and complex needs, supporting admission avoidance and high-quality care closer to home. The ACP will provide strategic clinical leadership across the care home service and contribute to service transformation.

The ACP will provide expert clinical decision making for undifferentiated presentations, lead complex case management, review high acuity patients with complex needs and support end-of-life care, and work collaboratively with GP practices, community teams and care home providers to improve outcomes for residents, reduce avoidable hospital admissions and facilitate care closer to home.

The postholder will be required to support, supervise and coach staff within the Care Home Team to develop skills, competency and identify training needs across the team etc

Review pathways of Care and streamline processes within the PCN Care home.

About us

The Lytham St Annes Primary Care Network covers 5 GP Practices, Holland House, Fernbank, Parcliffe, Poplar, and Ansdell, serving a population of approximately 53,000 registered patients. The PCN serves a diverse population with relatively low deprivation with a complexity from an above average elderly population and above average residents in care homes.

The PCN has a large population that reside in Care/ Nursing Homes with approximately 36 care homes across the footprint and 900+ residents.

Operational Lead for the PCN Care Home Team

Provide day to day oversight of the PCN Care Home Team, managing demand on a daily basis and assigning visits etc to the most appropriate Clinician within the team

Clinical leadership for the Care Home Service across the PCN, acting as an expert resource, role model and supporting care for complex patient care.

Quality Improvement (QI): Identify, design, and implement clinical audits and QI projects to enhance the quality, safety, and efficiency of care home services across the PCN.

Risk Management -support any investigation that may occur such as, clinical incidents, near-misses, and safeguarding concerns within care homes, ensuring lessons are learned and embedded into local practice

Communicate sensitive/contentious and occasionally highly complex medicine related information and advice to patients, carers, GPs and other members of the health care team.

Communicate on a variety of different levels depending on whether you are dealing with health professionals or patients. Information may be complex and require translation into a more patient friendly format.

Advanced Clinical Practice & Acute Response

Participate in multidisciplinary reviews of high-risk residents to optimise health outcomes and reduce avoidable deterioration.

Develop personalised care and support plans in partnership with residents, families, care home staff and GP practices.

Facilitate advance care planning and DNACPR discussions with residents and families.

Support residents with frailty, dementia, multiple long-term conditions and complex healthcare needs.

Provide clinical leadership for multidisciplinary discussions relating to high-risk residents.

Support residents to remain within their preferred place of care wherever appropriate.

Enable residents approaching end of life to receive care within their care home where this reflects their wishes and clinical needs.

Clinical Governance and Quality

Lead clinical governance arrangements for the Care Home Service, ensuring delivery of safe, effective and evidence-based care.

Work collaboratively with GP Clinical Leads and Care Home Managers to review quality, outcomes and clinical risks.

Lead investigations into incidents, complaints, safeguarding concerns and significant events. ensuring relevant safety systems are used to report and monitor incidents for quality improvements

Ensure learning from governance activity is embedded into clinical practice and service delivery.

Monitor standards of prescribing, record keeping and clinical documentation.

Develop, implement and review care pathways, protocols and standard operating procedures.

Lead and participate in clinical audit and quality improvement programmes to improve resident outcomes.

Ensure compliance with professional, regulatory and organisational standards.

Report incidents on Ulysees and support any investigations that may occur such as clinical incidents, near misses and safeguarding concerns within care homes or the PCN service, ensuring lessons are learned and embedded into practice both locally and within the wider NHS if required.

Education

Act as a clinical supervisor, mentor and educator for trainee ACPs, nurses, paramedics, physician associates and other healthcare professionals.

Deliver education, training and competency development programmes for care home staff and multidisciplinary colleagues.
