Job summary

We are recruiting an established practice nurse to join our friendly team at Oakwood and Bull Farm Surgery.

The successful candidate will work with the Nursing team and GP Partners in delivering preventative nursing interventions and management of long term conditions to all patients.

Main duties of the job

The post holder is responsible for the delivery of practice nursing services and care for the practice population. They will deliver care within the boundaries of their role, focusing on supporting patients to be healthy, managing of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients and supporting the delivery of policies and procedures.

Working pattern to be agreed.

The successful candidate will be required to work across both our main site: Church Street, Mansfield Woodhouse, NG19 8BL and our branch site: Bull Farm, Concorde Way, Mansfield, NG19 7JZ.

Will require additional enhanced access working hours on a Friday evening until 8pm and Saturday morning on a rota basis.
