Job summary

An exciting opportunity has arisen for a Bank Band 6 Single Point of Access Clinician to join the Single Point of Access service at Priestley Wharf. The postholder will provide expert clinical triage and assessment for patients with acute and long-term conditions, helping to prevent avoidable hospital admissions and support safe, timely discharge from hospital. • Initiate investigations, interpret diagnostic results and plan, manage and evaluate care where appropriate, with support from senior clinicians.
  • Make referrals for medical assessment and diagnostic procedures within agreed pathways.
  • Signpost and refer patients to appropriate health and social care services to reduce unnecessary A&E attendances and hospital admissions.
  • Support safe discharge into community settings.
  • Communicate complex and sensitive information clearly and compassionately to patients, carers, families and professionals.
  • Maintain accurate, complete and contemporaneous clinical records.
  • Contribute to safeguarding, infection prevention, clinical governance, audit and service improvement.
