<p>As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This is a rewarding opportunity to deliver care where it matters most, in peoples homes and local clinics, helping individuals stay <span class="single_desc_int_gb_word">independent and <span class="single_desc_int_gb_word">supported</span> within their own communities</span>. This role is ideal for someone who enjoys working out in the community rather than being based in one location all day. You will build meaningful relationships with patients and play a key role in <span class="single_desc_int_gb_word">supporting their health and wellbeing in a more personal, one to one setting</span>.
