Job summary
We are looking for an Activity coordinator who is passionate, motivated and willing to join our team in providing recovery focused support for individuals with mental health needs.
This post is for 16 hours a week, fixed term for 6 months.

Main duties of the job
- To provide interventions with personal recovery principles in order to improve the service users' quality of life and to support them to do the things they want to do and live the life they want to lead.
- To facilitate a programme of social, creative and skills-based activities to both individuals and groups which promotes social inclusion for service users.
- To work closely with the Multi-Disciplinary Team (MDT) in leading on the facilitation of client centred activities.

The postholder will work alongside colleagues in planning a high-quality programme of therapeutic and occupational activities for patient care and be involved in ward-based delivery of these as well as in the community, linking with wider statutory agencies and voluntary organisations.
To work within the integrated care team to encourage effective team working, implementing and maintaining high professional standards of care within the team and acute services.As a member of the multi-disciplinary team to be responsible for the assessment of need and the planning, implementation and evaluation of individualised care plans.To offer both individual and specialist group activities. To deliver direct patient care, according to individual care plans and in response to identified need.
