Job summary
St Johns Way Medical Centre is seeking a Digital Transformation Lead to support the safe and effective use of digital systems across the Practice. The postholder will improve patient access and staff workflows, use EMIS Web data to inform service improvement, and help colleagues make confident use of our systems. The role will work closely with Practice staff, PCN colleagues, NHS partners and digital suppliers.
Main duties of the job
1. Supporting practices to adopt new technology that improves patient access and experience
2. Reviewing and improving digital maturity across the Practice and PCN
3. Using data to understand demand, capacity, and activity, and to drive service improvement
4. Working within Integrated Neighbourhood Teams alongside general practice, community services, social care, and voluntary organisations
5. Building strong working relationships across practices and wider system partners
6. Supporting governance, risk management, and reporting linked to transformation and digital projects

St Johns Way Medical Centre is a well-established NHS GP practice located in Archway, serving a diverse local population of 12,000 with a strong focus on accessible, high-quality primary care. Technology Adoption and Patient Access
- Support the implementation and effective day-to-day use of the Practices digital systems to improve patient access, experience and staff workflows.
- Support relevant NHS and local digital initiatives, checking that changes are workable for the Practice and improve access to services for patients.
Practice Collaboration
- Build relationships and facilitate collaboration between member practices to support the delivery of care to each other's patients, for example through enhanced access or other hub-type working arrangements
Digital Maturity
- Review and improve the SJW's digital maturity
- Work with the Integrated Care System (ICS) and other partners to ensure that digital and operational Practice transformation is embedded in, and aligned with, ICS and national strategies
Data and Insight
- Use EMIS Web searches and reports to understand demand, capacity and activity, check data quality, and identify practical improvements to patient access and Practice operations.
- Use data to support population health management and improvements in care quality and practice operational efficiency
- Understand the type and intensity of support needs across the Practice and coordinate this support, including through organisational development (OD) programmes
- Present data insights to support service improvement and reduction in unwarranted variation
Neighbourhood and Integrated Working
- Work within Integrated Neighbourhood Teams (INTs), collaborating with general practice, community services, social care, and voluntary organisations
- Support delivery of place-based, population health-focused care, and contribute to reducing health inequalities and improving access
- Participate in multi-disciplinary team (MDT) meetings and neighbourhood forums as required
Stakeholder Engagement and Communication
- Build strong working relationships across PCN member practices and system partners
- Communicate digital changes clearly to staff and patients, and plan and deliver practical training, coaching and follow-up support to build staff confidence and skills. Identify training needs and arrange further upskilling where needed.
Governance and Quality
- Ensure digital activity follows the Practices governance, information governance and data protection requirements.
- Support clinical IT safety and digital risk management, recording and escalating safety concerns, incidents and supplier issues through the appropriate Practice processes.
- Monitor and report on performance against relevant PCN and DES objectives, maintaining accurate documentation
Working Conditions
- Travel across GP practices, community settings and care homes as service delivery requires
- A combination of office-based, remote, and on-site working
