Job summary

The post holder will support the delivery of the Prostate Cancer Supported Self-Management pathway, working closely with the Prostate Clinical Nurse Specialist (CNS) team to ensure the provision of safe, effective, and patient-centred care.

They will act as a key point of contact for patients on remote monitoring and active surveillance pathways, coordinating PSA tracking, MRI surveillance, routine follow-up investigations, and safety-netting processes in line with agreed treatment protocols.

Working autonomously within agreed professional and organisational guidelines, the post holder will support patients with self-management and ongoing follow-up care, referring to the Clinical Nurse Specialist team when additional clinical input is required. They will coordinate care by undertaking delegated clinical and administrative tasks, contributing to improved patient experience, pathway efficiency, and continuity of care.

The post holder will work collaboratively with the wider multidisciplinary team to ensure patients receive timely access to appropriate services and support throughout their cancer journey.

By providing proactive monitoring, coordination, and patient support, the post holder will play a vital role in enhancing patient outcomes, promoting wellbeing, and supporting the efficiency and effectiveness of the Prostate Cancer Service.

Main duties of the job

The post holder will be an integral member of the Urology Team, providing support to the Clinical Nurse Specialists (CNSs) in delivering high-quality, patient-centred care. The primary purpose of the role is to assist patients in navigating the complexities of the health and social care system during and following cancer treatment.

Working autonomously within established professional and organisational guidelines, the post holder will support patients with self-management and ongoing follow-up care, seeking guidance from the Clinical Nurse Specialist team when appropriate. They will play a key role in coordinating care, providing both emotional and practical support to individuals living with and beyond a cancer diagnosis.

The role will involve monitoring patients on active surveillance pathways, ensuring timely completion of routine blood tests, MRI scans, and other investigations in accordance with agreed treatment protocols. Reviewing patients within a set criteria and escalating when required.

Through effective communication and coordination, the post holder will contribute to improving the patient experience, promoting wellbeing, and ensuring that patients receive appropriate support throughout their cancer journey.
