Job summary

An exciting opportunity for a registered nurse to join a busy, forward thinking GP practice in Selby. We have a vacancy for a Practice Nurse to join our supportive nursing team

We have an incredibly supportive team of 3 Partners and 6 salaried GPs, and you will work within the nursing team. Nursing duties will involve the care of those patients referred by a GP and also those groups of patients whose care is directly arranged by the post holder.

You will be a Registered Nurse and have the required level of training and competence. You will be expected to work with patient group directives and practice protocols, where these are available, and to understand the aims, objectives and health plan of the practice. You must have a current NMC identification number and current membership of a recognised professional defence organisation.

Main duties of the job

The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to the practice population, working as part of the multidisciplinary team to promote health, prevent disease and support the management of acute and long-term conditions.

Key duties include undertaking clinical assessments and providing treatment room services such as wound care, venepuncture, cervical cytology (where qualified), ECGs, immunisations, injections, removal of sutures and clips, and specimen collection. The postholder will administer medicines and vaccines in accordance with PGDs, PSDs and practice protocols.

The role includes the management of patients with long-term conditions including diabetes, asthma, COPD, hypertension and cardiovascular disease, undertaking routine monitoring and reviews and supporting patients to self-manage their health.

The Practice Nurse will contribute to health promotion and disease prevention through lifestyle advice, NHS Health Checks, screening and immunisation programmes and public health initiatives. Additional responsibilities may include women's health, family planning, travel health, spirometry and Doppler assessments, subject to training and competence, and respond appropriately to medical emergencies

The postholder will maintain accurate clinical records, adhere to infection prevention and control standards, fulfil safeguarding responsibilities, participate in clinical governance and maintain professional competence through CPD.

You will work as a member of the practice nursing team and will also be a fully participating member of the full Primary Health Care Team. Nursing duties will involve the care of those patients referred by a GP and also those groups of patients whose care is directly arranged by the post holder.

You will be a Registered Nurse and have the required level of training and competence. Clinical responsibilities

Provide skilled nursing care to patients through agreed protocols both face to face and via the telephone

Work with other members of the nursing team to ensure that the practice has the appropriate level of nursing cover at all times.

Maintain competence in anaphylaxis and resuscitation techniques

Take responsibility for the care of agreed groups of patients and be able to work independently

Maintenance of accurate documentation and records utilising IT as appropriate and in accordance with practice protocols

Facilitate specific disease management clinics as agreed with GPs

Chronic disease monitoring and annual review

Maintain reflection on own clinical practice, contributing to clinical governance

The delegation of duties appropriately and timely to other members of the Practice Nurse team

  • Ensure the clinical computer system is kept up to date with accurate details recorded and amended, actively engaging in the Practices commitment to the Quality and Outcomes Framework (QOF) process
  • Participate in annual appraisals
  • Attend staff meetings and briefings as required
  • Comply with national and local health and safety legislation and instructions
  • Act as mentor to more junior members of the team and work with students allocated to the Practice.
