Job summary

Following investment into the community hospitals wards we have an exciting opportunity to join our team. We are seeking a highly motivated and enthusiastic Band 6 Physiotherapist to join our therapy rehabilitation team based in the North locality. The base site will be Weardale Community Hospital. 30 hours per week.

Applicants must be prepared to participate in a 6 day week service, covering bank holidays and Saturdays on a rota basis.

The successful applicant will be required to work across the North and South Community Hospitals in the Trust as and when required.

Main duties of the job

his post is for a Specialist Physiotherapist, 30 hours per week.

The service will work over 6 days a week.

The successful applicant will independently manage a caseload in the community wards providing holistic, short term, goal led intervention following a comprehensive assessment. These patients will present with a wide range of complex problems and will allow you to develop a highly specialist knowledge

Promoting a Discharge to Assess model of care, patients will have their discharge facilitated by therapists at the most optimum point in their rehabilitation pathway.
