Phoenix Health Primary Care Network
Salary: £22,100 per annum (equivalent to approximately £17.00 per hour)
Hours: 25 per week
Base: Working across Unity Health, Chinnor, Princes Risborough, Long Crendon, Thame and Brill

About the Role
Phoenix Health PCN is seeking an experienced and motivated PCN Operations & Workforce Lead to play a pivotal role in supporting the delivery of high-quality primary care services across our network.
Working closely with the Clinical Director, PCN Manager and Practice Managers, you will provide operational leadership, workforce coordination and project support to ensure our services are delivered effectively and efficiently. This is an excellent opportunity for someone with experience in healthcare operations, workforce planning, or project management who is passionate about improving patient care.
You will lead on workforce planning, support the development and deployment of our multidisciplinary workforce, oversee service improvement projects, and provide meaningful performance analysis to support informed decision-making across the network.

About Phoenix Health PCN
Phoenix Health PCN brings together three GP practices across Buckinghamshire working collaboratively to deliver more coordinated, accessible, and proactive healthcare for our local communities.
Our multidisciplinary team includes Health and Wellbeing Coaches, Clinical Pharmacists who support safe and effective medicines management, Pharmacy Technicians who assist with repeat prescribing and medication reviews, Care Coordinators who help patients navigate health and social care services, and Paramedics who provide timely assessment and urgent care within the community.
As demand grows and the needs of our population become increasingly complex, we are committed to strengthening our workforce further. We aim to expand our team with dedicated professionals who can help us provide tailored support to our most vulnerable patients such as those who may struggle with long-term conditions, frailty, social isolation, or barriers to accessing care. Support the implementation of PCN priorities and strategic objectives.
Develop and improve systems and processes to enhance efficiency across the network.

Identify operational risks and support timely resolution.
Lead and support quality improvement and service development initiatives.
Workforce Planning & Development
Lead workforce planning activity across the PCN.
Coordinate the effective deployment and utilisation of ARRS-funded roles.
Monitor workforce capacity, productivity and utilisation.
Support recruitment, onboarding and retention initiatives.
Work closely with practices to identify workforce gaps and opportunities for improvement.
