Job summary

The Senior Homecare Nurse is responsible for delivering high-quality clinical care whilst providing day-to-day leadership, support and line management to Clinical Nurses within the Baxter Homecare Programme.

This is a hybrid role combining approximately 60% direct patient care and clinical delivery with 40% leadership, operational and people management responsibilities. The post holder maintains an active clinical caseload and is expected to perform the full range of Clinical Nurse responsibilities whilst acting as a clinical expert, role model, coach and mentor to colleagues.

Working in partnership with the Clinical Nurse Manager, Senior Clinical Nurse Manager(s) and Clinical Account Managers, the Senior Homecare Nurse supports the delivery of safe, effective and patient-centred care whilst ensuring the achievement of contractual objectives, service standards and key performance indicators (KPIs).

The role plays a key part in promoting clinical excellence, developing nursing capability, supporting operational delivery and ensuring a seamless patient and stakeholder experience across the Baxter Homecare Programme.

Main duties of the job

  • Maintain an active clinical caseload and deliver high-quality, safe and effective patient care in accordance with programme requirements, Service Operating Procedures (SeOPs) and professional standards.
  • Deliver programme-specific clinical interventions within patient homes, community settings and virtual environments as required.
  • Conduct comprehensive patient assessments and identify changes in patient condition, escalating concerns appropriately.
  • Support patient onboarding, treatment initiation and ongoing clinical management.
  • Deliver patient, family and carer education to promote independence, confidence and treatment adherence.
  • Provide clinical support and guidance to patients, carers and healthcare professionals throughout the patient journey.
  • Support discharge planning and continuity of care in collaboration with NHS teams, carers and Clinical Account Managers.
  • Manage complex patients and provide specialist clinical guidance to colleagues where required.
  • Communicate effectively within multidisciplinary teams to ensure coordinated and patient-centred care.
  • Act as a clinical role model, promoting excellence in patient care, patient experience and professional practice.
  • Support the achievement of contractual KPIs, service objectives and clinical outcomes.
