Job summary
Smart General Practice builds "technology that understands practice": AI tools designed by GPs and built by NHS practices to take pressure off general practice. Our mission is to intelligently triage patients and remove the frustrations at the practice front door. We were founded by practising GPs and collaborated with receptionists and administration staff such as Pharmasists to provide our services to real problems.
You will design, build and run production software across the SMART platforms, from AI voice pipelines and triage APIs to the web front ends practices and patients use. You report to the CTO and work alongside the engineers who built and grew the platform to where it is today.
At SFIA Level 4/5 you work autonomously under general direction, own complex features end to end and lead reviews of colleagues' work. As you move to Level 5/6 you take technical responsibility across all stages of development for a product or service, set and monitor engineering standards, and give authoritative guidance to the team and the CTO.
This is a hands-on role in a small, fast-moving team. What you ship reaches real patients and real surgeries, often within the same sprint.
The salary for the role sits between £60,000-£70,000 per annum, depending upon experience.
Main duties of the job
- Estimate and plan work in sprint planning, and deliver against agreed sprint goals
- Build and integrate AI features using LLMs, speech-to-text and text-to-speech, including prompt design, tool calling and evaluation
- Design and maintain secure REST APIs and integrations with GP clinical systems and NHS partners
- Deploy and run services on Microsoft Azure using CI/CD, infrastructure as code, monitoring and alerting
- Resolve live operational issues and bugs in services that patients rely on
- Write clear technical documentation in Confluence and support clinical safety (DCB0129) and information security (ISO 27001, DSPT) evidence
- Apply version control and change control to protect work and keep releases traceable
About us
You will join a small engineering team that reports to the CTO and works side by side with GPs, practice staff and our clinical leadership. Our Chief Medical Officer is also our Clinical Safety Officer, and our COO leads information security, so clinical, security and engineering decisions are made in the same room. The engineers you will work with built SMART Voice, SMART Navigation, SMART Consult and SMART Med Check, and they know every part of the platform.
The atmosphere
We work inside a real GP practice in Bramley, Leeds. When a feature ships, you can walk down the corridor and see receptionists and clinicians using it. It is a startup in pace and a healthcare organisation in purpose: informal, collaborative and direct, with no layers between you and the people who make decisions. We are proud of what we build and share it openly, including speaking about SMART Voice at VapiCon 2026.
How we approach the work
- We ship. SMART Voice went from first build to answering real patient calls in around two months, and practices go live within a week.
- We own it. Engineers take features from design to production and stay with them when they are live.
- We are safe by default. Patient safety and data protection are part of the definition of done, not an afterthought.
- We are honest about capacity. We plan in sprints, track velocity and say no to work that would compromise quality.
- We learn together. Code reviews, pairing and retrospectives are how we raise the bar as a team.

Job description
Job responsibilities
Our SMART platform includes:
- SMART Voice: AI telephony that answers, triages and logs patient calls, live in NHS GP surgeries
- SMART Navigation: intelligent clinical triage that gets patients to the right person first time, delivered as a secure API to clinical system partners
- SMART Consult: next-generation e-consultations over video and messaging that write back into the patient record
- SMART Med Check: safety-netting that gives patients clear guidance if their symptoms change
The role You will design, build and run production software across the SMART platform, including AI voice pipelines, triage APIs and the web front ends that practices and patients use. You will report to the CTO and work alongside the engineers who built and grew the platform.
At SFIA Level 4 you will work on your own under general direction, own complex features from start to finish, and lead reviews of colleagues work. As you progress to Level 5 you will take technical responsibility for every stage of development on a product, set and monitor engineering standards, and give authoritative guidance to the team.
Key responsibilities (SFIA 9 Programming/software development, PROG)
- Design, build, test, document and restructure complex programs, scripts and integration services
- Help select development methods, tools, techniques and security practices
- Apply agreed standards, tools and security measures to produce well-engineered, secure software
- Review your own work and lead code reviews; use test-driven development, good test coverage and pair programming
- Support and guide other engineers, delegating where appropriate
- Build and integrate AI features using LLMs, speech-to-text and text-to-speech, including prompt design, tool calling and evaluation
- Design and maintain secure REST APIs and integrations with GP clinical systems and NHS partners
- Deploy and run services on Microsoft Azure using CI/CD, infrastructure as code, monitoring and alerting
- Resolve live operational issues and bugs in services that patients rely on
- Estimate and plan work in two-week sprints and deliver against sprint goals
- Write clear technical documentation and support clinical safety and information security evidence
- At Level 5: plan and drive software construction, monitor adherence to standards, and contribute to development policies and guidelines
Our technical environment
- Cloud: Microsoft Azure (App Service, Functions, SQL Server, Communication Services, Entra ID)
- AI: Azure OpenAI, Azure AI Foundry, LLM agents with tool calling, Deepgram and ElevenLabs
- Voice:, Twilio, SIP trunking, SMS
