Job summary
We have an exciting opportunity for a Band 3 Rehabilitation Assistant to join our multidisciplinary therapy team, delivering community-based rehabilitation to people with a range of conditions under the supervision of qualified therapists.
This is a full-time, permanent role, working Monday to Friday, 08:30-16:30. You will manage an allocated caseload of clients with specific, predictable presentations, providing rehabilitation and supporting people to achieve the best possible outcomes and independence.
You will also provide non-clinical support to qualified therapists and work closely with colleagues across community and acute teams, GPs, social services and voluntary sector services to support coordinated patient care.
The role includes lone working in line with Trust and local team policy.
Please note: we are unable to offer sponsorship for this Band 3 role.
Main duties of the job
You will deliver planned rehabilitation interventions under the supervision of a qualified therapist, supporting clients in their own homes, care homes and the wider community, including with outdoor mobility and community access.
You will manage an allocated clinical caseload, undertake holistic assessments and contribute to goal-focused treatment planning and interventions. You will monitor clients' progress and report any deterioration or significant change to the relevant therapist, discussing adjustments to rehabilitation programmes through structured clinical supervision.
Working in partnership with clients, carers and the multidisciplinary team, you will support a coordinated and individualised approach to rehabilitation, respecting clients' choices and helping them develop confidence in self-management.
You will communicate sensitively and effectively, maintain timely and accurate patient and equipment records, provide reports where required, attend relevant meetings and training, and contribute to the development of the Maximising Independence Team and patient pathway.
